RESOLUTION 02-155
APPROVING CHANGE ORDER #5 AS SUBMITTED BY
FEH ASSOCIATES,INC.
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,Change Order #5,submitted by FER Associates,Inc.includes:
1)Delete the 400Al3P non-fusible disconnect switch serving humidifier HUM-l
per Proposal Request 10 item no.1;Deduct $180.00
2)Add a 16"XI6"ceiling access panel at Men's restroom per Proposal Request
10 item no.2;Add $180.00
3)Provide 1Y."diameter PVC conduit for future power to the Butterfly Garden
area per Proposal Request 11 item no.2;Add $431.63
4)Extend 4"PVC conduit for telephone service entrance from west per Proposal
Request 11 item no.3;Add $337.85
5)Revise layout of Mechauical/Electrical equipment room to accommodate
equipment supplied to project.Work performed on a time and materials cost
basis per revised layout dated July 3,2002;Add $9,018.32
WHEREAS,a review of Change Order #5 by City Staff indicates that all items are
acceptable as charged except item #5 totaling $9,018.32;AND,
WHEREAS,this item appears to have been a result of design or construction of the
building rather than a result of a request by the Library Board to change mechanical or
electrical equipment.
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in
session this 2151 day of October 2002 that it hereby approves Items Numbered 1 through 4
totaling $769.48 in Change Order #5 as submitted by FER Associates,Inc.and excludes
from approval Item Number 5 totaling $9,018.32.
