MEETING DATE: September 21, 2020
AGENDA ITEM:Consideration of approval of Payment Estimate No. 4 to All Star Concrete, for
the Glynn Village Park and Trail Amenity project in the amount of $5,985.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Payment Estimate No. 4 to All Star Concrete is for
engineered wood fiber and grading and demo. The quantities completed to
date have been reviewed and agreed upon by the contractor, City of Waukee
Staff, and Genus Landscape Architects. The playground installer will move
from the Alice Nizzi Park Project to Glynn Village project next week and he
anticipates the install taking approximately a 7 days.

Dear Mr. Jermier:
Please find Construction Pay Application No. 4 in the amount of
$5,985.00 for the above-referenced project. The work completed
to date has been reviewed and agreed upon by the contractor,
City inspector and Genus. I hereby recommend that the City of
Waukee approve Construction Pay Application No. 4 as
presented by All Star Concrete.
