AGENDA ITEM:Consideration of approval of Change Order No. 1 to Concrete Connections,
Inc., for the Sugar Creek Park Project in the amount of $4,235.00.
FORMAT:Consent
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 to Concrete Connections, Inc. is for
the labor and materials to export excavated waste soils from the project area to
an adjacent project. During grading activities, additional soil was generated
that could not be wasted on-site, approximately 550 cubic yards, and was
hauled off-site. The unit price is $7.70 per CY, bringing the total cost to
$4,235.00. Representatives from Bolton & Menk, Inc. and City Staff have
reviewed the quantities and find them to be accurate.

Dear Mr. Jermier:
Submitted for your approval is Change Order (CO) #1 for the above-mentioned project. CO #1 includes
the labor and materials to export excavated waste soils from the project area to an adjacent city project.
During grading activities, additional soil was generated that could not be wasted on-site, approximately
550 cubic yards (CY), and was hauled off-site. The unit price is $7.70 per CY, bringing the total cost to
$4,235.00.
