SYNOPSIS INCLUDING PRO & CON: This quote is for demolition located at 500 N.
Warrior Lane including
1. Mobilization to and from the site.
2. Septic abandonment and disposal, water to be capped at the property line only. (Not the main)
3. Demolition and disposal of house and outbuildings on property.
4. Demolition and disposal of associated footings and foundations.
5. Capping and DNR Reporting of 2 wells located on property.
6. Asbestos abatement.
The City received 3 quotes from J. Pettiecord, Inc., Elder Corporation and DeCarlo Demolition. J.
Pettiecord submitted the apparent low quote in the amount of $44,250.00.
Based on review of the quotes received, Staff recommends award of the quotation for the
demolition located at 500 N. Warrior Lane to J Pettiecord, Inc. of Bondurant, IA in the amount of
$44,250.00.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: $44,250.00. This cost will
be included as part of the overall project cost for the Douglas Pkwy Culvert and Pond
Improvement project.

That the following quote for certain public improvements described in general as 500 N.
Warrior Lane Demolition Project and described in the Request for Quotes that was provided to
vendors be and is hereby accepted, the same being the lowest responsible quote received for such
work, as follows:

Scope of work will include:
1. Mobilization to and from the site.

Note:
• Does not include tree removal and/or tree debris removal. All trees to remain on site.
• Buildings are to be empty and free of hazardous chemicals. If there are any hazardous chemicals found
then they will be handled on a T&M basis.
• All trees and tree debris on property to remain.
• Asbestos abatement to be completed to asbestos found under NESHAP testing re quirements provided by
the report from the City of Waukee. Any additional asbestos found not listed on the report during
abatement and or demolition activities will be handled via change order.
• Owner to supply J. Pettiecord, Inc. with a copy of the asbestos survey and copies o f asbestos removal
records if needed.
• All salvaged material shall be retained by J. Pettiecord, Inc. (Including scrap metal.)
• Onsite material to be used as fill material, includes soil mixed with gravel site will be graded to drain .
• No importation of backfill materials.
• All testing, surveying, permits, additional mobilizations, seeding and erosion control measures, dewatering
of excavations is excluded.
• The attached image represents what is covered by this quote.
• Cost estimate is valid for 30 days.

House Demolition Scope:
DeCarlo Demolition Company submits its estimate for demolition work at 500 N. Warrior Lane in
Waukee, IA with the following scope of work:
o Disconnection of the water line at the north end of the lot line
o Closing of the two wells on the property
o Pumping and removal of the septic tank
o Abatement and disposal of the ACM per report from Iowa Environmental Services dated 2-24-22
o Filing of the 10 NESHAP notification to the Iowa DNR
o Obtaining of the demolition permit
o Mobilization and demobilization of manpower and equipment to and from the site
o Demolition, removal, and disposal of the 1,952 SF 1.5 Story House
o Demolition, removal, and disposal of the 960 SF Foundation, Slab, and Footings
o Balancing of the open excavation utilizing materials onsite
o Demolition, removal, and disposal of the 484 SF Detached Garage
o Demolition, removal, and disposal of the 484 SF Garage Slab and Footings
o Demolition, removal, and disposal of the 560 SF Pond Shack and 364 SF Deck
o Demolition, removal, and disposal of the foundation and footings
o Demolition, removal, and disposal of the 2,856 SF Morton Building
o Demolition, removal, and disposal of the 2,856 SF Slab

DeCarlo Demolition Company estimates to complete the above demolition scope of work to be $72,650.00.

Bid Exclusions: Work not specified in above demolition scope. Private utility locates. Disconnection or Relocation of utilities.
Asbestos inspection, testing, abatement, air monitoring, and filings. Additional materials not visible are subject to change order.
Evacuation of any HVAC units of gasses. Disposal of any oils, chemicals, hazardous material, or contaminated soils. Removal of
underground tanks, wells, cisterns, or sewer systems. Shoring, temporary enclosures, weatherproofing, patch back or repairs to
any surfaces. Storm water pollution prevention plan. Installation of silt fence, sock, or any other erosion prevention materials.
Backfilling of excavations. Importation/Exportation of fill material. Exportation of subbase materials. Soil testing and
compaction testing. Removal of drive approaches or city sidewalks, or paving shown as staying. Bonding or additional insurance
requirements. LEED Certification goals. Salvaging of items for the owner. Work stoppages due to others. Damage or
replacement of trees, turf, sidewalks, paving surfaces, irrigation system, etc. Fencing of the site.
Demo Notes:
• Owner to provide access to and from the site with limited disruption from others.
• Optioned work is to be done at the time of primary scope work. T&M charges to apply for additional mobilizations and
work.
• Work stoppages due to weather, others, or any other unforeseen events are subject to T&M charges.
• Any and all Federally announced Pandemics will extend the turnover date with no penalty.
• Any slab, foundation, walls, or footings with foam, plastic, coloring, overlay, etc. will be subject to change order if found to
be unable to recycle.
• Interior Slab removal includes removal up to 4” thickness.
• Exterior Slab removal includes removal up to 8” thickness.
• Footing removal includes up to 8’ wide by 8’ long and 4’ depth.
• Admin and tonnage fees will be charged if scale tickets are required for recycled materials
• This estimate is based on diesel fuel to cost below $4.00/gallon and landfill fees to not be 3% more than the cost of tipping
fees at the date of this estimate
• Means and methods to be discussed after execution of the contract
• T&M charges to apply if the means and methods used to bid the job are altered due to unforeseen circumstances
This Agreement shall be governed by and construed in accordance with the laws of the State of Iowa. In
the event that either party is required to initiate legal action to enforce the terms of this Agreement, said
action shall be filed in the Iowa District Court in and for Polk County, Iowa. The prevailing party in any
such action shall be entitled to the recovery of reasonable attorney fees and expenses – in addition to any
other remedies that may be available.
DeCarlo Demolition Company is to be held harmless for negligent acts of any sub-contractor. All salvage
becomes the property of DeCarlo Demolition Company upon receipt of signed copy of this proposal.
Please acknowledge your acceptance by signing and returning a copy of this proposal to our office.
Payments are due upon completion of work. Invoices not paid within 30 days are subject to a 1.5%
monthly finance charge.
Thank you for the opportunity to bid your project. Proposal will be in effect for thirty days.

NOTES: 1. EXCLUSIONS:
A. We exclude in our scope of work: bonds, permits and AGC dues, liquidated damages, cost of inspection,
layout staking/surveying, compaction tests, soil testing, saw cuts, damage to utilities or conduits embedded in
concrete, dewatering, abandoning or removal of intakes or manholes, temporary access roads, sheeting or
shoring, handling of contaminated soils, unacceptable soil removal or replacement, frost excavation, frost
removal, work when temps are 10 degrees or below, over excavation or recompaction, soil stabilization,
foundation or footing excavation, backfill of foundation walls, removal of foundation spoil, excavation or backfill
of retaining walls, handling of other contractor's soil, import or export of soil, import or export of granular
materials, import of topsoil or black dirt, under cutting fill areas, fine grading, hand excavation, grading for
sidewalks, repair to any work damaged by others, traffic control, hand tamping, silt fences, erosion control,
mowing, seeding or sodding, connection fees, pavement removal or replacement for utilities unless stated
above, boxout paving around intakes or steel plates to cover intakes and adjustment of fixtures, System
Development Fee, As-Built Drawing.
2. SPECIAL CONSIDERATIONS:
A. All service lines stop 5' away from building. Utility items tied to Earthwork items. All utilities backfilled with
trench spoils - Compaction to be with existing soil to 95% Standard Proctor. This Proposal may be withdrawn by
us if not accepted within 14 days. Payment to be made in monthly draws. Nothing shall obligate Elder
Corporation outside the scope listed herein.
B. We are not responsible for locating or notifying private utilities.
C. Contractor shall accept pad or parking grades prior to work done by other trades. Grading tolerances to be
within plus or minus two tenths of a foot.
3. The Agreement may be withdrawn by Elder Corporation without notice at any time prior to receipt by Elder
Corporation at its home office of a fully executed copy of same.
4. The laws of the State of Iowa shall apply to all claims and disputes arising out of or related to this Agreement
and/or the Project.
ACCEPTANCE OF PROPOSAL
We have carefully read and reviewed and hereby accept the foregoing Proposal for Construction Agreement
from Elder Corporation.
