AGENDA ITEM:Consideration of approval of Change Order No. 2 to Concrete Connections,
Inc., for the Sugar Creek Park Project in the amount of $2,684.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 to Concrete Connections, Inc. is for
the labor and materials to change out the meter socket to a meter with main
disconnect service and relocated to the exterior of the restroom building as
requested by the City of Waukee Building Department.

RE: Sugar Creek Park Change Order 2
Dear Mr. Jermier:
Submitted for your approval is Change Order (CO) #2 for the above-mentioned project. CO #2 includes
the labor and materials to change out the meter socket to a meter with main disconnect service and
relocated to the exterior of the restroom building as required by the City of Waukee Building Department.
The unit lump sum price for the work complete is $2,684.00.
Bolton & Menk, Inc. recommends the approval of CO #2. Please let me know if you have any questions.
