for the NW Douglas Parkway – NW 10th Street to NW 2nd Street Project
increasing the contract amount by $1,860.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 is for adding contract quantity to
correct Triumph Park driveway elevations.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $1,860.00.

Attached please find a detailed Change Order 2 for City Council approval on June 6, 2022.
Change Description:
• Item CO 7.11 Pavement Removals
o Add contract quantity to correct Triumph Park driveway elevations.
We have reviewed the submitted quotes from Elder Corporation and they appear to be in order
with current industry pricing. If City Staff concurs with this pricing, we recommend the City of
Waukee Increase the contract price in the amount of $1860.00 to complete the additional work
associated with Change Order #2. If you have any questions, please contact me.
