SYNOPSIS INCLUDING PRO & CON: J&K Contracting, LLC requested Payment Estimate
No. 6 (Final) for the work completed on the Little Walnut Creek North Trunk Sewer Extension -
Phase 2 Project, in the amount of $41,040.00. Veenstra and Kimm has reviewed the payment
estimate and recommends payment.
The partial payment is for the pay period December 1, 2021 to May 4, 2022. This pay estimate is
for erosion control and manhole internal drop connections.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: $41,040.00

CHANGE ORDER NO. 1
PAY ESTIMATE NO. 6 (FINAL)
Enclosed are three copies of Change Order No. 1 and Pay Estimate No. 6 (Final) for work on the Little
Walnut Creek North Trunk Sewer Extension – Phase 2 project, under the contract between the City of
Waukee and J&K Contracting, LLC dated May 3, 2021. The partial payment estimate is for the period
December 1, 2021 to May 4, 2022.
Change Order No. 1 is for adjustments to plan quantities to the as-constructed quantities.
We have checked the estimate and recommend payment to J&K Contracting, LLC in the amount of
$41,040.00.
Please sign all copies of Change Order No. 1 and Pay Estimate No. 6 (Final) in the space provided and
return one signed copy of the change order and the pay estimate to our office. Please forward one
signed copy of Change Order No. 1 and Pay Estimate No. 6 (Final) with payment to
J&K Contracting, LLC.
