SYNOPSIS INCLUDING PRO & CON: Change Order No. 4 includes work associated with
additional tree removal, temporary traffic control signing, and portable dynamic message signs.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $20,427.50.

UTE AVENUE AND ASHWORTH ROAD INTERSECTION PROJECT
Dear Rudy:
Enclosed is change order 04 for the Ute Avenue and Ashworth Road Intersection project. This
change order includes work associated with additional tree removal, temporary traffic control
signing, and portable dynamic message signs.
We recommend approval of change order 04 in the amount of $20,427.50 to the prime
contractor, Concrete Technologies, Inc. of Grimes, Iowa. Substantial completion date remains
unchanged with this change order. Please place this change order on the next available city
council meeting agenda for approval.

Trees are leaning over the roadway and it appears that they will eventually need to be removed. A adjustment to the unit price has been
made due to additional quantity over 20% of original.
Speed limits signs installed in an attempt to slow down traffic through the construction zone. Campground sign for enhanced business
access.
PDMS needed for advanced public warning above what was specified in the contract documents.
Clear and grub additional trees outside grading limits. Seven trees between 18-14 inch dia., three trees between 12-18 inch dia.
Provide and install six speed limit signs and one sign for Timberline Campground.
Provide two Portable Dynamic Message Signs (PDMS) for an additional 26 days.
