AGENDA ITEM:Consideration of approval of Change Order No. 3 to Concrete Connections,
Inc., for the Sugar Creek Park Project in the amount of $15,507.25.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 to Concrete Connections, Inc. is for
the labor and materials to change required for site restoration, erosion control,
and site establishment beyond the bid contract quantity. Also included within
the change order is the additional quantity for curb removal and replacement
required to match the existing street panels. The bid item quantity for silt
fence was not fully utilized and is shown as a quantity credit. All quantities
have been reviewed and agreed upon by City staff and Bolton & Menk.

RE: Sugar Creek Park Change Order 3
Dear Mr. Jermier:
Submitted for your approval is Change Order (CO) #3 for the above-mentioned project. CO #3 includes
the labor and materials required for site restoration, erosion control, and site establishment beyond the bid
contract quantity. Also included within the change order is the additional quantity for curb removal and
replacement required to match the existing street panel segments.
The bid item quantity for Silt fence was not fully utilized and is shown as a quantity credit on CO #3.
With the final revisions of Change Order #3, all project line items quantity additions and reductions are
quantified at a total dollar value of $15,507.25.
Bolton & Menk, Inc. recommends the approval of CO #3. Please let me know if you have any questions.
