AGENDA ITEM:Consideration of approval of Change Order No. 1 to J&K Contracting,
LLC for Little Walnut Creek Trunk Sewer Extension – Phase 5 Project
increasing the contract amount by $133,952.00.
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 is for the use of trenchless
sanitary sewer installation methods in an area with limited space for open cut. The reasons for the
change order is to ensure NW 10th St street pavement is protected, safety for the general public
and the contractor and to ensure the project remains on schedule. While excavating at nearly 35-
ft deep, the underlying soil conditions quickly deteriorated and the original intentions to open
trench this section of sanitary sewer on the East side of NW 10th St could not be done safety and
efficiently. The City is taking on approximately 45% of the cost and the Contractor is taking on
the remaining 55% of the total change order amount (approximately $470K).
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $133,952.00.

Enclosed are three copies of executed Change Order No. 1 for work on the Little Walnut Creek Trunk
Sewer Extension – Phase 5 project, under the contract between the City of Waukee and J&K
Contracting, LLC.
Please sign all copies of Change Order No. 1 in the space provided and return one signed copy of the
change order to our office. Please forward one signed copy of Change Order No. 1 to J&K Contracting,
LLC.
Change Order No. 1 is for the use of trenchless sanitary sewer installation methods in an area with
limited space for open cut.
