AGENDA ITEM:Consideration of approval of Change Order No. 2 to Absolute Concrete
Construction, Inc. for the Warrior Lane Improvements – Phase 2 project
increasing the contract amount by $2,024.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 is for the crosswalk signal
concrete base. The existing rectangular rapid-flashing beacon crosswalk signals on Warrior Lane
were salvaged to be reinstalled between Waukee Middle School and Waukee Public Library at
the new crosswalk location. The existing screw-in base could not be used due to an obstruction
underground, so a new cast-in-place concrete footing will be utilized instead.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $2,024.00.

Warrior Lane Improvements - Phase 2
Dear Mr. Koester:
Please find attached Change Order #1 for the Warrior Lane Improvements - Phase 2 project. The
change order includes the following items:
• Crosswalk Signal Concrete Base
The existing rectangular rapid-flashing beacon crosswalk signals on Warrior Lane were salvaged to be
reinstalled between Waukee Middle School and Waukee Public Library at the new crosswalk location.
The existing screw-in base could not be used due to an obstruction underground, so a ne w cast-in-
place concrete footing will be utilized instead.
We have reviewed the change order items and unit prices, and both are in line with anticipated
amounts. If City staff concurs with this change, we recommend the City of Waukee approve this change
order in the amount of $2,024.00 to the construction contract for the Warrior Lane Improvements –
Phase 2 with Absolute Concrete Construction, Inc. of Slater, IA.
