AGENDA ITEM:Consideration of approval of Change Order No. 2 to C.L. Carroll Co.,
Inc. for 2021 Well Rehabilitation Project – Phase 2 increasing the
contract amount by $26,986.01.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 is to increase the unit price for
the well pump due to price increases from ordering delays. The cost of this change order was
included in the damages collected from Northway Well and Pump Company for the 2021 Well
Rehabilitation Project’s late completion.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $26,986.01.

RECOMMENDATION: Approve the change order.

CHANGE ORDER NO. 2
Enclosed are three copies of executed Change Order No. 2 for work on the 2021 Well Rehabilitation
project – Phase 2, under the contract between the City of Waukee and C. L. Carroll Co., Inc. dated June
21, 2021.
Please sign all copies of Change Order No. 2 in the space provided and return one signed copy of the
change order to our office. Please forward one signed copy of Change Order No. 2 with payment to
C. L. Carroll Co., Inc.
Change Order No. 2 is for an increase in price of the well pump.
