AGENDA ITEM: Consideration of approval of resolutions approving a preliminary plat,
construction drawings, final plat, and site plan for Waukee Primary &
Specialty Care Clinic / Ashworth North Plat 2
FORMAT: Resolution
SYNOPSIS INCLUDING PRO & CON: The applicant is requesting approval of a preliminary plat,
construction drawings, final plat and site plan for a 6.98-acre commercial
development located north of SE Ashworth Road and east of Grand Prairie
Parkway.
The project involves the platting of one lot for commercial development for
the construction of a two-story medical office building. The building is 55,000
square feet in area and 29-feet in height. Two accesses will be provided into
the site. One off of Grand Prairie Parkway and one off of SE Tallgrass Lane.
The applicant has provided elevations for the proposed building on site. The
design and materials are consistent with the Site Plan Ordinance.
The construction drawings include the extension of a public street just to the
north of the proposed lot as well as extensions of public utilities that will serve
the proposed project.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS:
COMMISSION/BOARD/COMMITTEE COMMENT: The Planning and Zoning Commission
reviewed the preliminary plat, final plat and site plan request at their
meeting on September 13, 2022 and recommended approval:
Senior Planner, Melissa DeBoer, introduced the request for approval of a preliminary plat, final plat,
and site plan for a medical office building as submitted by the applicant, UnityPoint Health. The
property is generally located north of SE Ashworth Road and east of Grand Prairie Parkway,
containing approximately 6.98 acres.
Mrs. DeBoer advised that the project involves the construction of a two-story medical office building
approximately 55,000 square feet in area and 29 feet in height. A trash enclosure is located at the
northeast corner of the building and two monument signs are proposed at the entrances off of Grand
Prairie Parkway and SE Tallgrass Lane. The site also indicates a future expansion that would require
separate approval at that time. Public utilities will serve the site, and stormwater management is
accommodated in an off – site detention basin. All parking, landscaping, open space, and elevation
requirements have been met at this time.
Staff would recommend approval of the preliminary plat, final plat, and site plan subject to remaining
staff comments and review of the legal documents.

• Commissioner Broderick questioned the size of the expansion. A representative of the
applicant advised a total of 20,000 square feet is proposed for the addition at a later date.
Commissioner Hoifeldt moved to approve a Preliminary Plat for Waukee Primary & Specialty Care
Clinic subject to remaining staff comments and review of the legal documents. The motion was
seconded by commissioner Koelker. Ayes: Koelker, Hoifeldt, Streit, and Broderick. Recused: Tessau.
Nays: none. Motion Carried.
Commissioner Hoifeldt moved to approve a Final Plat for Ashworth North Plat 2 subject to remaining
staff comments and review of the legal documents. The motion was seconded by commissioner
Koelker. Ayes: Koelker, Hoifeldt, Streit, and Broderick. Recused: Tessau. Nays: none. Motion
Carried.
Commissioner Hoifeldt moved to approve a Site Plan for Waukee Primary & Specialty Care Clinic
subject to remaining staff comments and review of the legal documents. The motion was seconded by
commissioner Broderick. Ayes: Koelker, Hoifeldt, Streit, and Broderick. Recused: Tessau. Nays:
none. Motion Carried.
STAFF REVIEW AND COMMENT: Staff recommends approval of the preliminary plat,
construction drawings, final plat, and site plan subject to remaining staff
comments.
RECOMMENDATION: Approve the resolutions.

approval of a Preliminary Plat for Waukee Primary & Specialty Care Clinic on September 13,
2022; AND,
WHEREAS, the Preliminary Plat for Waukee Primary & Specialty Care Clinic is in general
conformance with section §176 - Subdivision Regulations of the Waukee Municipal Code.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee, Iowa on

WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, construction drawings have been submitted to the City for Waukee Primary &
Specialty Care Clinic; AND,
WHEREAS, the construction drawings have been reviewed by the City Engineer and City staff,
and are found to be in general conformance with section §177 – Subdivision Regulations –
Design Standards of the Waukee Municipal Code.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee, Iowa on
this 3rd day of October, 2022 that the Construction Drawings for Waukee Primary & Specialty
Care Clinic be approved.

WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, AP NORTH, LC, has requested final plat approval of Ashworth North Plat 2 (the “Plat”);
AND,
WHEREAS, the City of Waukee Planning and Zoning Commission recommended approval of the Plat
on September 13, 2022, subject to remaining staff comments and review of the legal documents; AND,
WHEREAS, the Final Plat for Ashworth North Plat 2 and the documents relating thereto will comply
with section §176 – Subdivision Regulations of the Waukee Municipal Code.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Waukee, Iowa on this
3rd day of October, 2022, that the Final Plat for Ashworth North Plat 2 be and the same is hereby
accepted and approved by the City Council of the City of Waukee.
BE IT FURTHER RESOLVED that the City of Waukee does hereby accept the following easements

PROJECT DESCRIPTION
The project involves the construction of a two-story medical office building. The building is 55,000 sq ft in area
and 29-ft in height. The main entrance to the building is shown along the south side of the site with a canopy.
The site plan also indicates a potential for a future expansion of the building that is 20,000 square feet in area.
Any proposed expansion would require site plan approval at that time.
A trash enclosure is located at the northeast corner of the building. Two monument signs are proposed. One
at the entrance off of Grand Prairie Parkway and one at the entrance off of SE Tallgrass Lane.
The final plat includes one lot. The lot is 6.98 acres in areas.
ACCESS AND PARKING
Two accesses are proposed. One off of Grand Prairie Parkway and one off of SE Tallgrass Lane.
A 10-ft wide trail exists just to the west side of the site along Grand Prairie Parkway. A 5-ft wide sidewalk will
be installed along the south side of SE Tallgrass Lane. A 5-ft wide sidewalk will provide access into the site
from the public sidewalk off of SE Tallgrass Lane. A 5-ft wide sidewalk will also provide access into the site
from the public trail to the south. Several internal sidewalks are shown to provide pedestrian accessibility
across the site.
A total of 165 parking spaces are required for the proposed site plan and they are providing 362 spaces. Per
the Kettlestone Design Guidelines a total of 11 bicycle parking spaces are required. They are providing the
required bicycle parking spaces.
UTILITIES
Public utilities will be provided to the site. Storm water management will be provided with a detention basin
located off-site.
LANDSCAPING & OPEN SPACE
A minimum of 15% of the project area is required to be open space. The total amount of open space
proposed is 33.9%. The landscape plan meets the requirements of the Landscape and Open Space Ordinance.

ELEVATIONS
The elevations of the building are proposed to be constructed mainly of fiber cement panels, metal wall panels,
and glass.
COMPREHENSIVE PLAN
The subject property is classified as Business Park which includes office or related uses, either arranged in
formal “Business Park” settings or developments, or in smaller concentrations located throughout Waukee.
The typical project size ranges from 15,000 to over 100,000 square feet and 1 to 6 stories in height. Proximity
to major road corridors is an important locational factor.
STAFF RECOMMENDATION
The preliminary plat, final plat and site plan are in general conformance with the Zoning Ordinance,
Subdivision Ordinance, Site Plan Ordinance, Kettlestone Design Guidelines, and Comprehensive Plan. Staff
recommends approval of the preliminary plat, final plat, and site plan subject to remaining staff comments and
review of the legal documents.
