This change order includes work associated trenchless vs. trenched water main construction east of Sugar Creek and removal of a temporary fire hydrant that is no longer needed. We recommend approval of change order 05 in the amount of $18,860.00 to the prime contractor, Concrete Technologies, Inc. of Grimes, Iowa. Substantial completion date remains unchanged with this change order. Please place this change order on the next available city council meeting agenda for approval. Contractor 1001 SE 37th Street Address Grimes, IA 50111 City, State, Zip You are directed to make the following changes in this contract: 1.Description of change to be made: Remove temporary installation of fire hydrant that was added via change order no. 2. Install water main crossing at Sugar Creek per original as-bid design. Install water main from Ashworth Road station 144+00 to station 149+44.67 by trenchless methods. 2.Reason for Change: Temporary installation of fire hydrant to facilitate Xenia water main abandonment is no longer needed. Based on subcontractor request, water main alignment/profile was revised at the Sugar Creek crossing, this was conveyed with design revision 4. Prior to water main installation, storm sewer and street construction was completed for the Timber Ridge subdivision. Trenched installation of water main would result in removal/replacement of newly constructed subdivision infrastructure. Further coordination resulted in a request from the subcontractor to install water main per the original as-bid alignment/profile and continue trenchless methods from the creek crossing to the end of the project.
