FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 4 is for a Century Link vault patch
repair, lane closure for extra curb and gutter work, and extra way finding signs.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $16,196.27.

RECOMMENDATION: Approve the change order.

Dear Ms. Kappos:
Attached please find a detailed Change Order for City Council approval on December 19, 2022.
Change Description:
• Item CO 11.11 Century Link Vault Patch Repair
o Additional Compensation for the pavement repair over the Century Link vault
upgrades under Ashworth.
• Item CO 11.12 Lane Closure for Extra Curb and Gutter Work
o Additional Compensation for additional traffic control for extra work out to Grand
Prairie Parkway.
• Item CO 11.13 Extra Way Finding Signs
o Additional Compensation for extra signage to help the public find the businesses
located within the road closure.
We have reviewed the submitted quotes from Alliance Construction and they appear to be in
order with current industry pricing. If City Staff concurs with this pricing, we recommend the City
of Waukee revise the contract price in the amount of $16,196.27 to complete the additional work
associated with Change Order #4. If you have any questions, please contact me.
