Project increasing the contract amount by $5,290.00.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 6 includes work associated with
removal of an existing air release manhole and installation of geogrid due to subgrade conflict
with a CenturyLink utility.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $5,290.00. The amount of $2,500 for
geogrid installation has been invoiced to Century Link.

UTE AVENUE AND ASHWORTH ROAD INTERSECTION PROJECT
Dear Rudy:
Enclosed is change order 06 for the Ute Avenue and Ashworth Road Intersection project. This
change order includes work associated with removal of an existing air release manhole and
installation of geogrid due to subgrade conflict with a CenturyLink utility. It is understood the
cost associated with geogrid installation have been invoiced to CenturyLink.
We recommend approval of change order 06 in the amount of $5,290.00 to the prime contractor,
Concrete Technologies, Inc. of Grimes, Iowa. Substantial completion date remains unchanged
with this change order. Please place this change order on the next available city council meeting
agenda for approval.

It was determined that an existing manhole was no longer needed and should be removed instead of vertical adjustment.
Geogrid was needed due to conflict between proposed subgrade and CenturyLink utility. Cost for the extra work will be invoiced to
CenturyLink.
Delete one major manhole adjustment.
Remove existing air release manhole.
Furnish and install geogrid at CenturyLink utility conflict.
