SYNOPSIS INCLUDING PRO & CON: The original agreement approved April 18th, 2022 with
Foth was for preliminary survey, functional design, and a traffic impact study. Amendment 1 is to
have Foth staff continue services to complete preliminary and final design, Public Safety site survey,
bid services, and construction services for the T Avenue Improvements. The T Avenue Improvements
also includes grading and site utilities on the Public Safety Site with the intent to receive lower unit
prices for both projects specific to grading and underground utility installation.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: Amendment 1 increases the
contract amount by $801,000.00. The revised contract amount including amendment 1 will be
$1,201,000.00 which is approximately 11% of the total project cost estimated at $11.16M.

DOUGLAS PARKWAY]
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA
WHEREAS, the City of Waukee, Dallas County, State of Iowa, is a duly organized Municipal
Organization; AND,
WHEREAS, on April 18, 2022, the City retained the services of Foth Infrastructure and
Environment, LLC for the T Avenue Corridor Improvements, W Hickman Road to NW Douglas
Parkway (RES 2022-168); AND,
WHEREAS, the scope of services is amended to include additional services to complete
preliminary and final design for the roadway and Public Safety site, site survey for the Public
Safety site, bid services, and construction services amending the contract fee to $1,201,000.00;
AND,
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that
Amendment No. 1 to the Agreement for Professional Consulting Services between Foth
Infrastructure and Environment, LLC and the City of Waukee, Iowa [T Avenue Corridor
Improvements, W Hickman Road to NW Douglas Parkway] is hereby approved.

ENGINEERING SERVICES AMENDMENT NO. 1
PROJECT NO. 22W007.00
THIS AMENDMENT, made and entered into this day of 2023 by and between,
the CITY OF WAUKEE , IOWA, hereinafter referred to as the “City”, and FOTH INFRASTRUCTURE AND
ENVIRONMENT, LLC., hereinafter referred to as the "Consultant", for professional services in connection with the
preliminary survey and mapping, functional design, for the proposed improvements of T Avenue Corridor Improvements,
from W Hickman Road to NW Douglas Parkway, hereby amends the original Agreement dated April 18th, 2022 as follows:
The City and Consultant agree to amend the original Agreement to include additional services associated with:
• Additional Project Coordination to finalize adjacent Easements
• Additional Project Coordination for Utility Relocations
• Additional Design Survey Services to incorporate adjacent Public Safety Building
• Design Services through Final Design of Phase 1 construction
• Construction Admin and Survey Services through construction of Phase 1
• Phase 1 Construction consists of the following:
o Sanitary Sewer trunkline along T Ave from Hickman Rd to north of NW Douglas Pkwy
o Water Main along T Ave and NW Douglas Pkwy
o Northbound lanes of T Ave and Eastbound lanes of NW Douglas Pkwy adjacent to Public
Safety Building
o Full build Storm Sewer system adjacent to Public Safety Site
o HMA overlay from Hickman Rd to just south of Public Safety Site
o HMA overlay south of Hickman Rd to just south of new turn lane
o Signal and turn lane improvements to Hickman Rd and T Ave
o Pedestrian Underpass at NW Douglas Pkwy with associated trail reconstruction
I. Basic Services of the Consultant
The Consultant shall consult on a regular basis with the City to clarify and define the City’s requirements for the Project
and review available data. The City agrees to furnish to the Consultant full information with res pect to the City’s
requirements, including any special or extraordinary considerations for the Project or special services needed, and also to
make available pertinent existing data. The Consultant shall provide the following basic services in regard to the Project:
A. PUBLIC SAFETY SITE SURVEY
Task 1 – Project Control
Establish supplemental horizontal and vertical control for the Project area base d on the City permanent
Horizontal and Vertical Control monuments. Each permanent control point or benchmark sha ll have horizontal
coordinates or elevation, recovery information, and monument description. Accurate descriptions of the
horizontal control points and benchmarks will be created and recorded on the plan sheets.
Task 2 - Topographic Survey
Perform topographic surveys required for the development of the Project. H orizontal and vertical accuracies
shall be adequate to produce a map that is compatible with the topographic data for incorporation into the
existing mapping.
Task 3 - Boundary
Perform property surveys required for the development of the project. The property surveys shall be in-depth
legal surveys for which acquisition plats are to be developed.

This task includes a thorough search of City, County, and State records to review all surveys of record pertaining
to the survey corridor, including County Auditor’s Subdivision Plats, original government surveys, early surveys
made by County Surveyors, all irregular land survey, and road establishment records. Copies of such records
are to be included in the project file for future reference.
This task also includes obtaining sufficient field data to locate or establish property lines affected by this project
to enable the preparation of the improvement plans. This includes locating section corners, property pins, and
visible lines of occupation such as fences, field divisions, or any other lines indicating possession. A diligent
effort must be made to recover existing land corner monuments necessary to describe the right-of-way along the
project corridor.
The task includes incorporation of property lines, right-of-way lines, and ownership of properties affected by the
project into the base mapping for the project. The bas e mapping will identify the owners and approximate
boundaries of all appropriate parcels within the survey limits.
Task 4 - Project Base Map
Incorporate field surveys into an elec tronic base map to be used for the design of the project. Incorporate
property lines, right-of-way lines, and property ownership for parcels located within the project limits into the
base map.
Task 5 – Preparation of Preliminary Plat and Final Plat
The Consultant shall prepare a Preliminary Plat consistent with the City of Waukee ‘Preliminary Plat Checklist’
and consistent with Land Subdivision Ordinance – Ch. 176.06 . The Final Plat shall be prepared as per the City
of Waukee ‘Final Plat Checklist’ and consistent with Land Subdivision Ordinance – Ch. 176.07.
Task 6 – Site As-built Surveys
Perform separate topographic as built surveys for initial rough site grading operations and final grading
operations after Public Safety Site Completion. Horizontal and vertical accuracies shall be adequate to produce
a map that is compatible with the topographic data for incorporation into the existin g mapping. It is not
anticipated to conduct as built surveys on underground utilities, buildings or paved features.
B. PROJECT COORDINATION
Task 1 – Maintain and Update Project Mailing List
The Consultant will maintain the project mailing list and update it as needed to include all local officials, agencies
and interested parties. The City will provide the initial list of interested parties.
Task 2 – Public Information Meetings
The Consultant will conduct one (1 ) public informational meeting(s) that will be attended by a minimum of three
(3) staff members of the Consultant. The purpose of the meeting will be to provide a brief overview of the
proposed improvements to the surrounding property owners. The meeting will include a discussion of the
construction plan, as well as gather information on the concerns, priorities and specific issues of the adjacent
property owners and other affected parties. This task includes preparation of the public notification letter, display
materials and hand out information for the meeting. The reserving of the meeting facility and a public notification
will be completed by the City.
Task 3 – Individual Property Owner Meetings
The Consultant will attend one (1) meeting with each of nine (9) property owners, if necessary. The meeting will
be attended by a minimum of two (2) staff members of the Consultant. The meeting will be coordinated by the
City’s property acquisition consultant. The purpose of the meeting will be to provide detailed information of the

proposed improvements and impacts to the property. The meeting will include exhibits and an active discussion
including any concerns. This task includes preparation of exhibits, the meeting(s) and follow-up.
Task 4 – Utility Coordination
The Consultant will conduct two (2) utility coordination meetings with representatives of various utility
companies. The first meeting will be to advise the utility of the nature and extent of the proposed improvements
and solicit mapping and input from the utility to develop an unders tanding of the order of magnitude and
limitations of their facilities located within the project corridor. The second meeting will be to review the collected
field data, identify any missing informa tion, share with each utility the proposed functional layout of the project,
and begin initial assessments of impacts to each utility. The Consultant shall document all communications and
correspondence with each utility company and share with the City prior to the start of preliminary design. Known
utilities in the corridor include:
• MidAmerican Energy Company
• Lumen
• Aureon Network Services
• Iowa Communications Network
• Northern Natural Gas
• Xenia
Task 5 – Periodic Meetings with Local Elected Officials
Informational update presentations will be provided to the City Council on an as needed basis during the design
development process. The presentations w ill provide an introduction/overview of the project and project goals
and objectives. It will also provide an update on design development concepts and schedule. The presentations
will be held in conjunction with the Council Work Sessions. F or estimating purposes, it is assumed that the
Consultant will attend two (2) meetings.
Task 6 – Exhibit Prepa ration for Council Meetings
Exhibits for the design development will consist of aerial mosaics for the project with the major project features
indicated on the exhibit. Include coloring, lettering, and other techniques to delineate the proposed design and
right of-way needs, including typical sections of the roadway. T he figure will indicate right-of-way needs, property
lines, property ownership, access control lines, structure limits, and roadway improvements.
Task 7 - Project Management
The project manager for the Consultant will be responsible for monthly progress reporting, minutes of meetings,
interoffice memoranda, and invoicing. This task a lso includes scheduling of staff, coordinating with
Subconsultants, review of progress, and senior review of de liverables.
C. T AVENUE ROADWAY SURVEY
Task 1 – Preparation of Pla ts
The Consultant shall prepare acquisition plats and legal documents for property to be acquired and needed for
construction easements for the project. For estimating purposes, it is assumed that one (1) plat for acquisitions,
thirteen (13 ) permanent easement and seven (7) temporary construction easements will be needed for this
project. The Consultant shall coordinate with the City for any remaining Real Estate Services required for the
project.
Individual plats and legal descriptions will be prepared for each parcel. The legal descriptions shall be metes &
bounds descriptions. The plats and legal descriptions shall comply with requirements of the Iowa Code and shall
be prepared by or under the direct supervision of a duly licensed surveyor under the laws of the State of Iowa.
The plats shall also be completed in accordance with the City of Waukee requirements.

scope of service task has been developed:

Task 2 – Staking for Real Estate Negotiations
This task consists of staking easement locations for purposes of negotiations. The staking survey includes the
marking of key easement location points to visualize the locations in the field and shall also include marking of
existing property line locations. For this agreement, all permanent and temporary easements will be staked one
(1) time for each property.
Task 3 – Permanent Monumentation of Right-of-Way
This task consists of monumentation of the right-of-way acquired as part of this project which will be performed
upon completion of construction of the proposed improvements. The monumentation will comply with
requirements of the Iowa Code and will be performe d by or under the direct supervision of a duly licensed land
surveyor under the laws of the State of Iowa.
This task also includes resetting existing right-of-way monumentation for those parcels with only a temporary
construction easement.
Task 4 - Utility Relocation Staking
Perform staking to aid in the relocation of utility relocations. This task will include communication with utility
companies regarding their needs and staking. This task includes (1) staking effort per each utility company.
The Consultant will maintain the project mailing.
D. PRELIMINARY DESIGN AND PLANS
Task 1 – Preliminary Design and Plan Preparation
Upon completion of the preliminary plans, the design plans will be approximately 60 percent complete. The work
to be performed by the Consultant under Preliminary Design shall consist of the following tasks:
Title and General Information Sheets (A Sheets)
This task consists of assembling the preliminary title and general information sheets. The preliminary title sheets
will include the following: Index of Sheets, Legend, Location Map, Project Number, Design Traffic data.
Preliminary Typical Sections (B Sheets)
Typical sections will be developed for the mainline street improvements. The typical sections will reflect lane
widths, curb section and type, sidewalk widths, trail widths, right-of-way widths, pavement types and clear zones.
This task does not include pavement and subgrade design.

This task also includes identifying the potential storm sewer, water main and other utility loca tions in the
development of the typical sections. The scope of services for this task does not include design calculations and
capacity analysis for storm sewer, water mains or sanitary sewer.
This task also consists of drafting typical cross sections and standard details to be utilized for the improvements.
Plan and Profiles (D and E Sheets)
Preliminary roadway geometrics, connecting roadway geometrics and pedestrian facility geometrics will be
developed for the project based on survey centerline establishment, preferred alternative conceptual geometrics
and approved SUDAS design criteria.
Utilizing the preliminary geometrics, the horizontal alignments and vertical profiles for the mainline, connecting
roadways and pedestrian facilities will be develop ed. The alignments and profiles will be based on the
established design criteria.
This task also consists of the development of preliminary pla n and profile sheets that will show the existing
topography along with the proposed improvements. Proposed Rig ht-of-Way and Construction Easement limits
based on the catch point lines for all roadways and pedestrian facil ities will be shown. Included will be the
necessary CADD work to show the preliminary design features for the proposed improvements.
Site Grading Layout (ED Sheets)
This item consists of the preliminary drafting of the Public Safety Site grading layout.
Traffic Control, Staging, Pavement Markings and Traffic Signing (J Sheets)
A preliminary traffic control concept will be developed to indicate h ow the project will generally be constructed as
well as maintenance of traffic through construction. The stagi ng plan will consider all elements of construction
including roadway, side roads, driveways, storm sewer, utility improvements and property acces s.
This task also consists of the development of sheets reflecting the staging plan for inclusion in the preliminary
plan set.
This task also includes the preliminary development of pavement markings and traffic signing plans to be placed
into service following construction. The traffic control devices, procedures, and layouts shall be as per the
Manual on Unifo rm Traffic Control Devices (MUTCD).
Storm Drainage Design (M Sheets)
Existing hydrologic and hydraulic conditions will be inventoried and impac ts of the proposed system analyzed
area-wide. Once the impacts have been quantified, the preliminary storm sew er layout will be developed and
sizing based on proposed improvements and existing drainage patterns utilizing Chapter 2 of the SUDAS Design
Manual. Resolve potential conflicts with underground utilities and other design elements. Vertical Profiles will
be developed as part of this task. The layout will be included in the plan view of the mainline plan and profile
sheets.
Potential conflicts with underground utilities should be resolved at this time.
Sanitary Sewer and Water Main Adjustment Design (MS and MW Sheets)
This task consists of the final design of the sanitary sewer and wate r main that will show the proposed
improvements, flow line e levations (where necessary,) rim elevations, utility access locations, hydrant and valve
locations.

Traffic Signal and Interconnect Plan (N Sheets)
This task consists of preliminary t raffic signal design at US 6/Hickman Road and T Avenue. The preliminary
plans will include traffic signal pole layout and major equipment layout. This task also includes gathering
information on the type of equipment to be used for the project using availa ble design standards of the City, and
the Manual on Uniform Traffic Control Devices (MUTCD).
The preliminary plans will include the interconnect layout of conduit and fiber. This task will include utility
coordination to ensure adequate spacing and conne ctions to existing services.
Sidewalk Plan (S Sheets)
Develop preliminary curb ramp layout and accessible curb ramp design in accordance with Chapter 12 of Iowa
SUDAS Design Manual. Design ramp geometric configurations alternatives, identify surface requirement, review
general horizontal curb openings, cross slopes, running slope, and identify sidewalk width and passing space
within the corridor.
Preliminary Culvert Plans (V Sheets)
This task includes the preparation of type, size and location of major r oadway drainage and pedestrian
structures. For estimating purposes, one (1) pedestrian underpass is assumed.
Preliminary Design Cross-Sections (W Sheets)
This item consists of the prelimina ry design and drafting of individual cross sections for the project. Cross
sections will be designed and drawn at 50-foot maximum intervals, with additional cross-sections included as
necessary. Cross sections will show the centerline, the existin g ground elevations as well as the final project
grading, fore slope and back slope information, special sub-grade treatments, ditches, pavement replacement,
and other pertinent information.
Task 2 - Preliminary Field Review
A field review will be held with the Project Development Team to discuss key issues and design concepts,
including drainage, access control, traffic control/stage co nstruction and right of-way. The review will determine
the completion of the plan design, identify needed adjustments to minimize potential property impact and confirm
the proposed staging plans. Revisions will be noted for preparation of the final design. Completion of the field
review will allow preparation for Final Plans.
Task 3 – Quality Control
Ongoing quality control input from the Project Team and the Consultant’s senior technical staff will occur
throughout the development of preliminary plans. T he design engineer is responsible for making specific
recommendations and ensuring that critical issues are discussed and resolved prior to submittal of the
preliminary plan set to the Project Team.
Review the preliminary engineering plan set for technical accuracy, as well as for general constructability and
conformance with the project design criteria.
Task 4 – Preliminary Opinion of Probable Cost
A preliminary opinion of probable construction cost for the project will be prepared. Preliminary cost estimates
will be based on representative major project elements and recent bid information. Detailed quantity takeoffs will
not be developed for the preliminary cost estimate and at times, percentages of overall budgeted cost may be
utilized to develop the estimate.
Task 5 – Budget Review
Using the Opinion of Probable Cost, the Consultant shall, if necessary, make recommendations pertaining to
modifications in the Project in order to address b udgetary concerns.

Task 6 – Monthly Project Status Meetings
It is estimated that the Consultant’s staff will attend four (4) meetings with the City, and others to review
progress, assist in decision-making, and receive direction from the City. It is understood by the parties that the
Consultant will attend additional meetings as needed to complete the Project.
Task 7 – Acquisition Determination
Identification of permanent right -of-way and permanent/temporary easement needs will be determined based on
the preliminary design development. The requirements will take into consideration the proposed improvements,
utilities, construction staging and access, utility relocation and other critical construction elements.
Task 8 – Preparation of Right-of-Way Exhibits
The Consultant shall prepare an exhibit that clearly and distinctly shows impacts to properties to in form the City
of the project impacts and to use during discussions with property owners.
Task 9 – Public Notice of Project
The Consultant shall prepare a project notice letter for property owners directly adjacent to the project. This letter
will provide a brief project description, notification of property surveys, anticipated sched ule and contact
information. The Consultant shall distribute the notic e letter following the City review and approval.
E. FINAL PLANS AND SPECIFICATIONS
Task 1 – Incorporate City Comments From Preliminary Plan Review
The Consultant will respond to comments resu lting from the preliminary plan review and field review.
Recommended modifications will be incorporated into the plan set. The Consultant will revise preliminary
construction plans and special provisions to incorporate appropriate comments.
Task 2 – Pre-Final (Check) Design and Plan Preparation
The Consultant shall provide the following deliverables:
Final Title Sheets (A Sheets)
This item consists of finalizing the title sheet. The title sheets will include the following: Index of Sheets, Legend,
Location Map, Project Number and Design Traffic Data.
Final Typical Sections (B Sheets)
This item consists of final design and drafting of typical cross sections and standard details to be utilized for the
improvements.
Final Quantities (C Sheets)
This item consists of final bid items to be included in the Project, as well as final quantity tabulations, and the
development of the general notes and estimate reference information.
Final Plan and Profiles (D and E Sheets)
This item consists of the final design and drafting of roadway plan and profile sheets, including the detail
information required for plan approvals, permitting, and co nstruction of the proposed improvements. This task
also consists of the final design and drafting for the installatio n of new utilities and utility adjustments that can be
determined from coordination with the utility companies at the time of design.
Site Grading Layout (ED Sheets)
This item consists of the final detailing of the Public Safety Site grading layout.

Final Survey Reference (G Sheets)
This task consists of assembling reference ties to the plan control points and the benchmark data used to
develop the plans and to be preserved throughout construction of the project. This task also includes preserving
any Government Corners that are found in the vicinity of the proposed construction.
Right-of-Way (H Sheets)
This item consists of final design and layout of any ROW or easements required to construct the project.
Final Traffic Control/Staging, Pavement Markings Traffic Signing (J Sheets)
This item consists of final design and drafting of the traffic control and staging plans.
This task also includes the finalization of the development of pavement markings and traffic signing plans to be
placed into service follow ing construction. It is assumed that the City will install the permanent signing for the
Project.
The traffic control devices, procedures, and layouts shall be as per the Manual on Uniform Traffic Control
Devices (MUTCD).
Final Geometric Staking, Jointing, and Edge Profiles (L Sheets)
This item consists of the final design and drafting of jointing details, spot elevations, and geometric layouts for all
non- typical pavement areas.
Final Design of Storm Drainage Systems (M Sheets)
This item consists of final design and drafting of storm sewers, storm sewer in lets, manholes, open ditches,
culverts, and other storm drainage related facilities for the Project. This item also includes the design and
drafting of storm profiles and cross sections.
Final Sanitary Sewer and Water Main Adjustment Design (MS and MW Sheets)
This task consists of the final design of the sanitary sewer and water main that will show the proposed
improvements, flow line elevations (where necessary,) rim elevations, utility access locations, hydrant and valve
locations.
Traffic Signal and Interconnect Plans (N Sheets)
This task consists of the final design and drafting of the traffic signal as well as the interconnect. Included in this
task will be the signal layout plans, wiring diagrams, major equipment, poles, signal heads, quantities, special
provisions, underground interconnect, and other elements of the traffic signa l installation, as necessary, to
accommodate the roadway design, as well as proposed phasing and initial timing s of the traffic signal.
Included in this task is coordination with the Iowa Department of Transportation for approval of the traffic signal
layout and operations.
Final Roadway Lighting Plans (P Sheets)
This task consists of selection of lighting poles and fixture types, design and drawing preparation of a final

The Consultant will work closely with the City and MidAme rican Energy Company to ensure that the proposed
lighting system is compatible with the overall project design and meets the needs of the City.

Final Erosion Control Plan (R Sheets)
This task consists of the development of erosion control sheets that show a draft pol lution prevention plan,
existing and proposed topography, location of proposed best management practices, and permanent surface
restoration types.
Final Sidewalk Plan (S Sheets)
Develop final curb ramp layout and accessible curb ramp design in accordance with Chapter 12 of Iowa SUDAS
Design Manual. Finalize ramp geometric configurations and calculate horizontal curb openings, cross slopes,
running slope, and label sidewalk width and passing space within the corridor.
Final Tabulation of Earthwork (T Sheets)
This task consists of final design and drafting of the soils information and earthwork quantities.
Final Construction Details (U Sheets)
This item consists of the final design and drafting of special project details not covered in other items. Inc luded
are such items as special grading details, channel grading, culvert details not included in the standard drawings,
special storm sewer or manhole details not included in the standard drawings, special paving details, and other
required miscellaneous details found to be required for completion of the project.
Final Culvert Plans (V Sheets)
This task consists of final design, calculations and drafting of the proposed pedestrian underpass under NW
Douglas Parkway, including situation plans and necessary construction details to accommodate construction.
Final Design Cross-Sections (W and X Sheets)
This item consists of the final design and drafting of ind ividual cross sections for the project. Cross sections will
be designed and drawn at 50-foot maximum intervals, with additional cross-sections included as necessary.
Cross sections will show the existing ground elevations as well as the final project grading, including foreslope
and backslope information, special sub-grade treatments, ditches, pavement replacement, and other pertinent
information.
Task 3 – Project Permitting
The Consultant will assist the City in preparing applications for permits from g overnmental authorities that have
jurisdiction to approve the design of the project and participate in consultations with such authorities, as
necessary. The Consultant will prepare the following document for the project:
• Iowa DOT Application to Perform Work Within ROW
• Iowa DOT Application of Approval for a Traffic Control Device (TCD Application)
• IDNR Water Supply Section, Construction Permit Application
• IDNR Wastewater Construction Permit Application
• IDNR NPDES Storm water Discharge Permit
• Notice of Publication
Any fees for construction permits, licenses or other costs associated with permits and approvals will be the
responsibility of the City. The Consultant will prov ide technical criteria, written descriptions and design data for
the City’s use in filing the applications for permits. The Consultant will prepare the permit applications and other
documentation.
Task 4 - Quality Control
Involve ongoing quality control input from the Project Team and the Consultant’s senior technical staff
throughout the development of Check Plans. The design engineer is responsible for making specific

recommendations and ensuring that critical issues are discussed and resolved prior to submittal of the Check
Plans set to the Project Team.
Review the Check Plans set for technical accuracy, as well as for general constructability and conformance with
the project design criteria.
Task 5 – Incorporate City Comments from Pre-Final (Check) Plans Submittal
The Consultant will respond to comments resulting from the Check Plans review. Recommended modifications
will be incorporated into the plan set.
Task 6 – Final Plan Submitta l
The Consultant will submit Final Plans.
Task 7 – Project Manual
This task consists of providing a Project Manual for the Project, including Notice of Hearing and Letting,
Instruction to Bidders, Bid Forms, Bond Forms, Agreement Forms, General Conditions of the Construction
Contract, Supplementary General Conditions, and Detailed Construction Specifications.
Task 8 – Opinion of Probable Construction Cost
The Consultant shall update a Final Opinion of Probable Construction Cost for the project at the tim e of
completion of the Check Plans. Detailed quantity takeoffs will be developed for the opinion of probable cost
F. LETTING SERVICES
Task 1 – Advertisement for Bids/Bid Phase
The Consultant shall assist in the preparation of the notice to Contractors. The Consultant shall disseminate
contract documents. The work tasks to be performed o r coordinated by the Consultant shall include the
following: sending informal notice of project to contractors, copying of plans and project manual, distribution of
plans and project manual, distribution of bid tabulation, preparation of construction contract documents and
arrange preconstruction meeting.
The work tasks to be performed or coordinated by the City during the Bid Period Services shall inclu de the
following: publishing the notice of hearing and letting (Consultant shall p repare the notice of hearing and letting).
Task 2 – Plan Clarification
The Consultant shall be available to answer questions from contractors prior to the letting and sh all issue
addenda as appropriate to interpret, clarify or expand the bidding documen ts.
Task 3 – Letting, Bid Tabs, and Award Recommendation
The Consultant shall have a representative present when the bids are opened, shall make tabulation of bids for
the City, shall advise the City on the responsiveness of the bidders, and assist the City in making the award of
contract. After the awards are made, the Consultant shall assist in the preparation of the necessary contract
documents.
G. CONSTRUCTION SERVICES
Task 1 – Construction Administration
Construction administration services shall consist of office based services to assist the City in implementing the
construction contract for this project. The work tasks to be performed by the Consultant shall include:

Pre-construction Meeting
The Consultant shall conduct a pre -construction meeting after award of construction contract for the City’s
Contractor, subcontractors, utility companies, and other interested parties.
Shop Drawings Submittal Reviews
The Consultant shall review shop drawings, samples, and other data which the Contractor is required to submit,
but only for conformance with design concept of the Project and conformance with the information given in the
contract documents. The Consultant shall evaluate and determine the a cceptability of substitute materials and
equipment proposed by the Contractor. The Consultant shall have authority to require special inspection or
testing of the work, and shall receive and review all certificates of inspections, testing, and approvals required.
Site Visits
In connection with observations of the Contractor’s work while it is in progress:
• The Consultant shall make visits to the site at intervals appropriate to the various stages of construction
as the Consultant deems necessary (estimated to be one (1) site visit per week) in order to observe as
an experienced and qualified design professional the progress and quality of the various aspects of the
Contractor’s work. Based on information obtained during such visits and on such observatio ns, the
Consultant shall determine, in general, if such work is proceeding in accordance with the Plans, and
the Consultant shall keep the City informed of the progress of the work.
• The purpose of the Consultant’s visits to the site will be to enable the C onsultant to better carry out his
duties and responsibilities during the construction phase and, in addition, by exercise of the
Consultant’s efforts as an experienced and qualified design professional, to provide for the City a
greater degree of confidence that the completed work of the Contractor will conform to the Plans, and
that the integrity of the design concept as reflected in the Plans has been implemented and preserved
by the Contractor.
• The Consultant shall not during such visits supervise, direc t, or have control over the Contractor’s work,
nor shall the Consultant have authority over or responsibility for the means, methods, techniques,
sequences, or procedures of construction selected by the Contractor for safety precautions and
programs incident to the work or for any failure of the Contractor to comply with laws, rules, regulations,
ordinances, codes, or orders applicable to the Contractor furnishing and performing his work.
• During such visits, the Consultant may disapprove of or reject the Co ntractor’s work while it is in
progress if the Consultant bel ieves that such work will not produce a completed Project that conforms
to the Plans, or that it will prejudice the integrity of the design concept of the Project as reflected in the
Plans.
Weekly Progress Meetings
The Consultant shall conduct weekly progress meeting with the City, Contractor, and Utility Companies to review
progress, update the project schedule, resolve conflicts, and determine any corrective actions if necessary . The
Consultant shall assist the Contractor in communicating upcoming construction activities with the adjacent
businesses and property owners.
Utility Coordination
The Consultant shall continue coordination, if necessary, for utility companies unable to relocate pr ior to the
commencement of construction. This task will include the coordination o f any utilities that were not identified
prior to construction.
Pay Requests and Change Order Preparation
The Consultant shall prepare contractor pay requests and issue nec essary interpretations and clarifications of
the Plans, and in connection therewith, prepare change orders as required.

Final Inspections and Project Close-Out
The Consultant shall conduct an inspection in the company of the City to determine if the work is substantially
complete and a final inspection to determine if the completed work is acceptable. The Consultant may
recommend, in writing, final payment to the Contractor and may give written notice to the City and the Contractor
that the work is acceptable. This task will also include assembling final project documentation, certifications, and
attendance at the project audits.
Record Drawings
This task consists of preparation of construction record drawings defining the actual location of improvements
and fixtures. The Consultant shall prepare record drawings showing those changes made during construction,
based on the marked-up drawings and other data furnished by the Contractor and the Resident Construction
Observer. The Consultant shall provide the City with the following:
• One (1) full size original of the record drawings.
• Digital version, half and full size, of record drawings
• Master CADD file
• Electronic Construction Documentation
o Approved Submittals
o Material Certifications
o Meeting Minutes
o Pay Requests
o Change Orders
o Certificate of Acceptance
The above Construction Administration Services is for the 2023 and 2024 Calendar Year only and includes a
maximum of 600 hours of effort. In the event the construction period exceeds the contract working da y or
unanticipated conditions require Construction Administra tion in excess of hours stated, the Consultant shall
notify the City as it approaches this limit and determine the additional effort to complete the project. The
Consultant and City shall work to develop a mutual resolution for the remaining effort
Task 2 – Construction Surve y
Task 1 - Project Control
Verify existing horizontal and vertical control as shown in the project plans and supplement with additional
control points as-needed to complete the project. Reset land corners and permanent reference mark ers as
indicated on the plans. Tie-ins with existing roadways shall be checked for correctness of alignment prior to
construction staking. This task includes resetting of all property corners disturbed by construction.
Task 2 - Property Lines and Temporary Easement
Stake temporary construction easements, permanent easements, and permanent right-of-way corners at 50 foot
intervals, or less if needed. These points shall be marked by placement of a metal pin or wood hub, flat, and lath
at the same location as the slope stakes.
Task 3 – Sanitary Sewer and Storm Sewer
Locations and elevations shall be marked with metal pin or tack in a wood hub, flat, and lath. The flat shall be
clearly marked with the station location, pipe number, cut/fill elevation to flo w line and form grade, and offset
distance to centerline of pipe.
Task 4 – Water Mains
Mark locations and elevations with metal pin or tack in a wood hub, flat, and lath. Clearly mark the flat w ith the

Task 5 – Intakes and Utility Accesses
Locations and elevations shall be marked with metal pin or tack in a wood hub, flat, and lath. The flat shall be
clearly marked with the station location; intake or utility acc ess number; cut/fill elevation, including bottom of well
and form grade; and offset distance to the Station Location.
Task 6 – Pavement (PCC)
a. Elevations (graded 4’ offset paving hubs) at 25 foot intervals for finished grading and paving.
b. At intersections, graded offset hubs will be set at each end of radius, and radius points will be set
where radius distance is less than 100 feet. Also graded midpoint offsets will be set on radii where
curve length is greater than 40 feet.
c. After grading and paving is complete, reset plan control points and land corner monuments.
*Note: the survey scope of services does not include blue-top staking for finished grading.
Task 7 – Sidewalk Ramps
The Contract Documents contain sheets for construction of curb ramps, turning s paces, and transitions. The
Contractor will measure or stake as required to construct features. If either of the following is met, the
Consultant will provide staking for that quadrant and verify slopes during finishing:
a) Running Slope. Tolerance between design slope and maximum allowable slope is less than 1.0%
b) Cross Slope, Turning Space Slopes (including areas adjacent to pedestrian pushbuttons). Tolerance of
+/- 0.5% from design would exceed minimum or maximum allowable slope.
Task 8 – Sidewalks
Locations and elevations shall be marked with a wood hub and flat. Elevations on one side of the pavement at
50 foot intervals on straight and level sections and at 25 foot intervals on horizo ntal and vertical curves. The flat
shall be clearly marked with the station location, cut/fill information, and offset distance to the edge of pavement.
The above Construction Survey estimate includes a maximum of 800 hours of effort. In the event the
Construction Period exceeds the contract working days or unanticipated conditions require Construction Survey
in excess of hours stated, the Consultant shall notify the City as it approaches this limit and determine the
additional effort to complete the project. The Consultant and City shall work to develop a mutual resolution for
the remaining effort.
PROJECT DELIVERABLES
The scope of services shall be considered complete upon completion and delivery of the following items to the satisfaction
of the City:
• Preliminary Design Plans

ADDITIONAL SERVICES:
Additional Services are not included in this Agreement. If authorized under a Supplemental Agreement the Consultant
shall furnish or obtain from others the following services:

to the Project
SCHEDULE
The Consultant shall complete the following phases of the Project in accordance with the schedule show n; assuming
notice to proceed is issued by the City on or before February 6, 2023.

the services, plus direct out-of-pocket costs for expenses incurred by personnel who are actually engaged in the work and
other direct costs. The total Hourly Not-to-Exceed fees for the professional engineering services as described in this
amendment are as follows:
