FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Change Order No. 5 is for additional compensation
for the relocation of the streetlight underground electrical in conflict with the Century Link vault
lid at 172+50.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $1,650.00.

RECOMMENDATION: Approve the change order.

Dear Ms. Kappos:
Attached please find a detailed Change Order for City Council approval on March 20, 2023.
Change Description:
• Item CO 11.14 Streetlight Relocation for Century Link Vault
o Additional compensation for the relocation of the streetlight underground
electrical in conflict with the Century Link vault lid at 172+50.
We have reviewed the submitted quotes from Alliance Construction and they appear to be in
order with current industry pricing. If City Staff concurs with this pricing, we recommend the City
of Waukee revise the contract price in the amount of $1,650.00 to complete the additional work
associated with Change Order #5. If you have any questions, please contact me.
