SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 includes work associated with
adjusting an existing sanitary sewer manhole to grade.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $3,704.80.

NW DOUGLAS PARKWAY – GRADE, PARTIAL PAVE AND JUT
Dear Sara:
Enclosed is change order 01 for the NW Douglas Parkway – Grade, Partial Pave and JUT
project. This change order includes work associated with adjusting an existing sanitary sewer
manhole to grade. This change is associated with coordination between the City and the Prairie
Rose development and includes a major adjustment to the structure.
We recommend approval of Change Order 01 in the amount of $3,704.80 to the prime
contractor, Concrete Technologies, Inc. of Grimes, Iowa. Substantial completion date remains
unchanged with this change order. Please place this change order on the next available city
council meeting agenda for approval.

Adjust existing sanitary sewer manhole to grade. Remove existing top portion of manhole, add barrel section and replace top.
2.Reason for Change:
Manhole needs to be raised to match proposed grading in the area.
3.Settlement for the cost of making the change shall be as follows:
