AGENDA ITEM:Consideration of approval of Change Order No. 2 to Concrete
Technologies, Inc. for the NW Douglas Parkway – Grade, Partial Pave
and JUT project increasing the contract amount by $14,997.90.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 2 is to adjust existing sanitary
sewer manhole to grade. Saw cut and remove two panels of roadway pavement. Excavate existing
manhole, remove existing top portion of manhole, add barrel section, replace top portion of
manhole, and backfill with rock. Full depth patch with 9” PCC pavement. Includes necessary
traffic control to perform work. Change Order No. 2 also modifies the contract completion date
from August 4, 2023 to August 11, 2023.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $14,997.90.

NW DOUGLAS PARKWAY – GRADE, PARTIAL PAVE AND JUT
Dear Sara:
Enclosed is change order 02 for the NW Douglas Parkway – Grade, Partial Pave and JUT
project. This change order includes work associated with adjusting an existing sanitary sewer
manhole to grade. The existing manhole was installed approximately 4-feet below grade to allow
agricultural use of the surface. The manhole needs to be raised to match proposed roadway
finished grade and is considered a major adjustment as it involves removal of pavement,
excavation, installation of an additional manhole barrel section, backfill and pavement
replacement.
We recommend approval of Change Order 02 in the amount of $14,997.90 to the prime
contractor, Concrete Technologies, Inc. of Grimes, Iowa. Seven calendar days will be added to
the contract end date to allow for this work, the substantial completion date is now August 11,
2023. Please place this change order on the next available city council meeting agenda for
approval.

1.Description of change to be made:

2.Reason for Change:
Existing manhole was installed approximately 4-feet below grade to allow agricultural use of the surface. Manhole needs to be raised
to match proposed roadway finished grade to allow maintenance access to the sanitary sewer.
3.Settlement for the cost of making the change shall be as follows:
