AGENDA ITEM:Consideration of approval of Payment Estimate No. 2 to Caliber Concrete,
LLC for the Heart of the Warrior Trail West Extension Project in the amount
of $4,050.75.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Payment Estimate No. 2 to Caliber Concrete, LLC is for
shared use path, PCC, 6 in., detectable warning panels, and painted pavement
markings. The quantities completed to date have been reviewed and agreed
upon by the contractor, City of Waukee Staff, and Shive-Hattery.

Dear Mr. Jermier:
Submitted for your approval is Pay Application 2 for the above-mentioned project. Pay Application 2
includes total work completed of $89,614.70 minus 5% retainage of $4,480.73 for a total payment of
$4,050.75. Please see Pay Application 2 for a full summary of the items completed that are included in
this payment.
