AGENDA ITEM:Consideration of approval of Change Order No. 1 to Caliber Concrete, LLC
for the Heart of the Warrior Trail West Extension Project in the amount of
$1,403.95.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 1 to Caliber Concrete, LLC is for final
construction quantity adjustment required for contract closeout. The change
order finalizes the total contract amount for the project. The approved
contract value of $88,210.75 will be increased $1,403.95 for a total final
contract value of $89,614.70. The quantities completed to date have been
reviewed and agreed upon by the contractor, City of Waukee Staff, and Bolton
& Menk, Inc.

Change Order 1
Dear Mr. Jermier:
Submitted for your approval is Change Order 1 for the above-mentioned project. Change Order 1
includes the final construction quantity adjustment required for contract closeout. The change order
finalizes the total contract amount for the project. The approved contract value of $88,210.75 will be
increased $1,403.95 for a total final contract value of $89,614.70. Please see Change Order 1 for a full
summary of the change items.
Bolton & Menk, Inc. recommends the approval of Change Order 1.
