AGENDA ITEM:Consideration of approval of Change Order No. 3 to Concrete
Technologies, Inc. for the NW Douglas Parkway – Grade, Partial Pave
and JUT project decreasing the contract amount by $30,465.88.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 includes adjustment of plan
quantities to match as-built conditions. It also removes the quantity of patching for the manhole
adjustment that was not properly consolidated.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order decreases the contract price in the amount of $30,465.88.

NW DOUGLAS PARKWAY – GRADE, PARTIAL PAVE AND JUT
Dear Sara:
Enclosed is change order 03 for the NW Douglas Parkway – Grade, Partial Pave and JUT
project. This change order includes adjustment of plan quantities to match as-built conditions. It
also removes the quantity of patching for the manhole adjustment that was not properly
consolidated.
We recommend approval of Change Order 03 which results in a net deduct to the contract in the
amount of ($30,465.88) to the prime contractor, Concrete Technologies, Inc. of Grimes, Iowa.
There is no change to the substantial completion date with his change order. Please place this
change order on the next available city council meeting agenda for approval.
