decreasing the contract amount by $92,865.25.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 4 is for adjustments to final
contract quantities to reflect actual item quantities used during construction.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order decreases the contract price in the amount of $92,865.25.

RECOMMENDATION: Approve the change order.

Warrior Lane Improvements - Phase 2
Dear Mr. Koester:
Please find attached Change Order #4 for the Warrior Lane Improvements - Phase 2 project. The
change order includes adjustments to final contract quantities to reflect actual item quantities used
during construction.
We have reviewed the change order items and unit prices, and both are in line with anticipated
amounts. If City staff concurs with this change, we recommend the City of Waukee approve this change
order deduct in the amount of ($92,865.25) to the construction contract for the Warrior Lane
Improvements – Phase 2 with Absolute Concrete Construction, Inc. of Slater, IA.
