AGENDA ITEM:Consideration of approval of Change Order No. 10 to Larson Construction,
Co., Inc., for the Triumph Park Project in the amount of $3,748.97.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 10 to Larson Construction, Co., Inc., is
an increase in the contract in the amount of $3,748.97. The work included in
change order #10 is work that was done in order to be able to have the
restrooms at the Miracle League Field open year-round in a conditioned
envelop (request from City Staff). There were multiple meetings with the
General Contractor and design team to determine what was deemed to be
rework and was deemed to be an added value to the City of Waukee that was
not included in the construction plans. Working with the City Building
official the added value items included cutting in a chase access panel,
purchase of the chase access panel, new lintel material, additional valves and
drain fittings, misc. fasteners, plywood, and copper pipe, and contractor labor
to complete this work. City staff have reviewed the quantities and find them
to be fair. This is the final Change Order for the project and upon approval of
accepting the public improvements the Triumph Park Construction project
will be closed out.

Triumph Park
Dear Mr. Jermier:
Attached please find Change Order #10 totaling a cost in the amount of $3,748.97, for approval and
signature by Waukee City Council. Work associated with this change occurred at a similar time as some
re-work by the contractor. There was some delay and several meetings to determine what was re-work
and what was added value to the City of Waukee.
The added value items included:
- Cutting in a chase access panel (cutting block and lintel)
- Purchase of the Chase access panel
- New Lintel material
- Additional valves and drain fittings.
- Misc. fasteners, plywood, and copper pipe
- Contractor Labor to complete this work.
The city, the design team, and the contractor have agreed that this change order only covers the work
that was added value to the project.
At this time, we believe there will be no more change orders to the project, and it can now be closed
out.
