AGENDA ITEM:Consideration of approval of Change Order No. 5 to All-Star Concrete,
LLC for the T Avenue Corridor Improvements – W Hickman Road to
NW Douglas Parkway Project increasing the contract amount by
$4,000.00.
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: The storm culverts at two driveways needed replaced
as they were taken out during sanitary sewer installation. Plans had 18” CMP already but not the
aprons. A total of 4 aprons will be needed for the two driveways.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $4,000.00.

RECOMMENDATION: Approve the change order.

Dear Mrs. Kappos:
Attached please find a detailed Change Order for City Council approval on April 15, 2024.
Change Description:
Storm culverts at the drives at 113+00 and 126+20 needed replaced as they were taken out
during sanitary sewer installation. Plans had 18" CMP already but not the aprons. Will need 4
each total for the two driveways.
We have reviewed the submitted quotes from All - Star and it appears to be in order with current
industry pricing. If City Staff concurs with this pricing, we recommend the City of Waukee
Increase the contract price in the amount of $4,000.00 to complete the additional work
associated with Change Order #5. If you have any questions, please contact me.
