[Ute Avenue & Ashworth Road Intersection Improvement Project] FORMAT:Resolution SYNOPSIS INCLUDING PRO & CON: The original agreement ($396,900) was approved by City Council on August 3, 2020. Supplemental Agreement No. 1 ($48,300) was approved by City Council on June 21, 2021. Supplemental Agreement No. 2 ($26,100) was approved by City Council on September 7, 2021. Supplemental Agreement No. 3 covers additional construction services provided by the consultant due to the contractor exceeding the contract completion date by 206 calendar days. Staff have included the cost of supplemental agreement no. 3 in the damages to be assessed to the contractor as a direct result of the project being completed past the contract completion date. FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: Amendment No. 3 is for an additional amount not to exceed $9,800.00. This amendment will bring the new contract amount to $481,100.00. [UTE AVENUE & ASHWORTH ROAD INTERSECTION IMPROVEMENT PROJECT] IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE, IOWA WHEREAS, the City of Waukee, Iowa is a duly organized municipality within Dallas County; AND, WHEREAS, on August 3, 2020 (Resolution #2020-292), the Waukee City Council approved an agreement with Snyder & Associates for Professional Consulting Services related to the Ute Avenue and Ashworth Road Intersection Improvement Project; AND, WHEREAS, on June 21, 2021 (Resolution #2021-274) and September 7, 2021 (Resolution #2022-373), the Waukee City Council approved Supplemental Agreements No. 1 and 2 respectively with Snyder & Associates for Professional Consulting Services related to the Ute Avenue and Ashworth Road Intersection Improvement Project; AND, WHEREAS, Supplemental Agreement No. 3 covers additional construction services provided by the consultant due to the contractor exceeding the contract completion date by 206 calendar days.
