AGENDA ITEM:Consideration of approval of Change Order No. 7 to All-Star Concrete,
LLC for the T Avenue Corridor Improvements – W Hickman Road to
NW Douglas Parkway Project increasing the contract amount by
$3,930.00.
FORMAT:Motion
SYNOPSIS INCLUDING PRO & CON: Detour routes from the T Ave project and the NW
10th St Crack and Seat project has conflicting signage, requiring sign modification to avoid
confusing drivers. The bike trail has had slippery conditions due to construction adjacent to the
trail, therefore adding signage to improve safety to trail users.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $3,930.00.

RECOMMENDATION: Approve the change order.

Dear Mrs. Kappos:
Attached please find a detailed Change Order for City Council approval on June 17, 2024.
Change Description:
The detour route from T Avenue and the 10th Street Crack and Seat project had conflicts between
the two. Modified some of the signage that showed conflicting directions to public. In addition,
the bike trail that runs through the project had two additional slippery when wet signs added for
increased safety.
We have reviewed the submitted quotes from All - Star and it appears to be in order with current
industry pricing. If City Staff concurs with this pricing, we recommend the City of Waukee
Increase the contract price in the amount of $3,930.00 to complete the additional work
associated with Change Order #7. If you have any questions, please contact me.
