AGENDA ITEM:Consideration of approval of Change Order No. 9 to All-Star Concrete
LLC for the T Avenue Corridor Improvements – W Hickman Road to
NW Douglas Parkway Project increasing the contract amount by
$8,255.00.
FORMAT:Consent
SYNOPSIS INCLUDING PRO & CON: Change Order No. 9 includes installation of an 8”
CMP culvert with flares needed to drain the road ditches at the west end of the 280th HMA tie-in
to existing gravel . Also included is additional traffic control needed for the work south of
Hickman on T Avenue to maintain a safe working and traveled way.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $8,255.00.

Dear Mrs. Kappos:
Attached please find a detailed Change Order for City Council approval on October 21, 2024.
Change Description:
A culvert is needed to drain the road ditches at the west end of the 280th HMA tie -in to existing
gravel . An 8" CMP with flares will be added to facilitate this. For the work south of Hickman on T
Avenue there was no traffic control noted for this work. Additional controls were added to
maintain a safe working and traveled way.
We have reviewed the submitted quotes from All - Star and it appears to be in order with current
industry pricing. If the City Staff concurs with this pricing, we recommend the City of Waukee
Increase the contract price in the amount of $8,255.00 to complete the additional work
associated with Change Order #9. If you have any questions, please contact me.
