the as-constructed quantities.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract amount by $84,274.00.

CHANGE ORDER NO. 4
Enclosed are three signed copies of Change Order No. 4 for the West Area Trunk Sewer Extension –
Phase 2 Project. Change Order No. 4 is to adjust the plan quantities to the as‐constructed quantities.
We recommend approval of Change Order No. 4 by the City Council of the City of Waukee.
Please sign all copies of Change Order No. 4 in the spaces provided. Return one signed copy of
Change Order No. 4 to our office and one signed copy to Rognes Corporation.
