FORMAT:Consent
SYNOPSIS INCLUDING PRO & CON: Change Order No.2 is for temporary traffic control
for flaggers, rock check dam, tile line repair, water main valve box repair/backfill, and 8” water
main fittings.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $7,847.85.

RE: Change Order #2 – Review and Recommendation to Approve
University Avenue Area Improvements - Phase 1
Dear Mr. Koester:
Please find attached Change Order #2 for the University Avenue Area Improvements - Phase 1 project.
The change order includes the following items:
• Temporary Traffic Control for Flaggers
• Rock Check Dam
• Tile Line Repair
• Water Main Valve Box Repair and Backfill
• Water Main Fittings – 8” 45° Bends
Temporary traffic control signage is added for flaggers on 6th Street/Ute Avenue to facilitate a lane
closure during paving of the 310th Street asphalt connection to 6th Street/Ute Avenue. Rock check
dams are added to the project to be used for erosion and sediment control in channel/ditch locations.
While attempting to shut down the existing 18" water main for connections to the newly-installed water
main, an existing water valve was found to be inoperable. Excavation, repair, and backfill of the existing
valve box is added so the existing valve could be operated to shut down the main and allow for
completion of the connections during the scheduled outage window. During excavation for this repair,
an existing drainage tile line was encountered and damaged, requiring repair. 8" 45-degree water main
bends are added to accommodate the elevation difference between the new 18" water main and the
existing 8" water main connection at the west side of the Waukee Public Works building.
We have reviewed the change order items and unit prices, and both are in line with anticipated
amounts. If City staff concurs with this change, we recommend the City of Waukee approve this change
order in the amount of $7,847.85 to the construction contract for the University Avenue Area
Improvements - Phase 1 with Elder Corporation of Des Moines, IA.
