AGENDA ITEM:Consideration of approval of a motion approving Change Order
No. 11 to All-Star Concrete, LLC, for the T Avenue Corridor
Improvements Project, W Hickman Road to NW Douglas Parkway,
increasing the contract in the amount of $36,123.80.
FORMAT:Consent
SYNOPSIS INCLUDING PRO & CON: Change Order No.11 is for correcting the redundant
credit to the city in the change order for the sign change addition and Gridsmart deduction under
the traffic signalization line item. The amounts were already accounted for with line item 8.20 and
8.21. Additional work was required near the properties east of T Avenue to handle stormwater
flows. This included export of soil and additional stone to dewater the area and the addition of an
area intake to take on surface flows.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $36,123.80.

Dear Mrs. Kappos:
Attached please find a detailed Change Order for City Council approval on August 4, 2025.
Change Description:
Corrected the redundant credit to the city in the change order for the sign change addition and
Gridsmart deduction under the traffic signalization line item. The amounts were already
accounted for with line item 8.20 and 8.21. Also, additional work was required near the properties
east of T Avenue to handle stormwater flows. This included export of soil and additional stone to
dewater area plus an intake to take on surface flows.
We have reviewed the quotes submitted from All - Star and it appears to be in order with current
industry pricing. If the City Staff concurs with this pricing, we recommend the City of Waukee
Increase the contract price in the amount of $36,123.80 to complete the additional work
associated with Change Order #11. If you have any questions, please contact me.
