AGENDA ITEM:Consideration of approval of Change Order No. 3 to Concrete Connection,
LLC., for Painted Woods West Park Project decreasing the contract in the
amount of $5,004.60.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 to Concrete Connection, LLC is for
net credit to the Painted Woods West project in the amount of $5,004.60. The
scope revisions include the addition of tree wells to improve tree health and
drainage and the removal of plantings in an area that was originally planned
for screening of a transformer that was removed from the project.

Please find Change Order #3 attached for review and consideration.
This change order is for a net credit to the Painted Woods West project
in the amount of $5,004.60. The scope revisions include:
1) Addition of tree wells to improve tree health and drainage.
2) Removal of plantings in an area that was originally planned for
screening of a transformer that was removed from the project.
Genus recommends that the City of Waukee approve Change Order
