AGENDA ITEM:Consideration of a motion approving Change Order No. 2 to
Holzworth Concrete for NW 6th Street Culvert Over Little
Walnut Creek, increasing the contract in the amount of $14,700.00
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No.2 is for grading the stockpile at the
NW Public Safety site to insure there is positive surface water drainage.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $14,700.00

Dear Mrs. Kappos:
Attached please find a detailed Change Order for City Council approval on April 13, 2026.
Change Description:
Holzworth graded around the stockpile at the public safety site to make sure there was positive
drainage and no ponding.
We have reviewed the submitted quotes from Holzworth and they appear to be in order with
current industry pricing. If City Staff concurs with this pricing, we recommend the City of Waukee
Increase the contract price in the amount of $14,700.00 to complete the additional work
associated with Change Order #2. If you have any questions, please contact me.
