AGENDA ITEM:Consideration of approval of Change Order No. 3 to Wendler, Inc., f/k/a
WRH, Inc., for the Lift Station Nos. 4 & 6 Rehabilitation Project,
increasing the contract amount by $12,437.57.
FORMAT:Consent Agenda
SYNOPSIS INCLUDING PRO & CON: Change Order No. 3 is for replacing both check
valves at Lift Station No. 6. Valve replacement was necessary after deficiencies were discovered
during construction.
FISCAL IMPACT INCLUDING COST/BENEFIT ANALYSIS: The net result of this
Change Order increases the contract price in the amount of $12,437.57.

LIFT STATION NO. 4 & 6 REHABILITATION
CHANGE ORDER NO. 3
Enclosed is a copy of Change Order No. 3 for the Lift Station Nos. 4 & 6 Rehabilitation project.
Change Order No 3 is for replacing both check valves at Lift Station No. 6. The check valves were
found not to be sealing and were allowing wastewater to flow backwards into the wet well.
Change Order No. 3 increases the contract price by $12,437.57.
We recommend approval of Change Order No. 3 by the City Council of the City of Waukee.
Please sign Change Order No. 3 in the space provided. Return one signed copy of Change Order
No. 3 to our office and forward one signed copy to Wendler, Inc.
