RESOLUTION 04-114 I
APPROVING ACCEPTANCE OF BID FROM MIDWEST UNDERGROUND
SUPPLY FOR A VACUUM UNIT IN THE AMOUNT OF $39,900
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,currently the Sanitary Sewer Department out sources the annual sewer
cleaning program;AND,
WHEREAS,this piece of equipment is to be used in this department as well as in other
departments for such projects as cleaning storm catch basins,setting street signs,
excavating and exposing other utilities,cleaning lift stations;AND,
WHEREAS,of the three bid notices mailed to vendors,the following were received:
Ditch Witch -Iowa,Inc.$30,125.00
Midwest Underground Supply,LCC $39,900.00;AND,
WHEREAS,Public Works Department Staff recommends purchasing the vacuum from
Midwest Underground Supply whose bid includes meeting all ofthe specifications
requested in the bid notice.

session this ih day of June 2004 that it hereby approves acceptance of the bid from
Midwest Underground Supply for a Vacuum Unit in the amount of $39,900.
