RESOLUTION 05-96
APPROVING CHANGES TO CITY OF WAUKEE HEALTH INSURANCE PLAN
OFFERINGS
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,The City of Waukee currently offers health insurance coverage to City
employees AND,
WHEREAS,the cost for health insurance has been increasing significantly in recent
years,AND,
WHEREAS,the City Council has a responsibility to Waukee taxpayers to provide the
required coverage in as fiscally responsible a manner as possible.
NOW THEREFORE BE IT RESOL YED by the City of Waukee City Council in
session this 6th day ofJune,2005 that it hereby approves a change to the City's health
insurance coverage effective July 1,2005 by replacing the current
Choice Standard Policy P10200l500POS80l03045 with the
Choice Standard Policy CSD25001 OOOPS103045C and the Premier Policy with
additional employee co-pay and deductible costs being self funded by the City of Waukee
and administered by a Third Part Administrator.
