APPROVING CHANGES TO CITY OF WAUKEE HEALTH INSURANCE PLA!'i
OFFERINGS
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
'WHEREAS,The City of Waukee currently offers health insurance coverage to City
employees AND,
WHEREAS,it is in the best interests of the City to reinstitute the prior health insurance
program AND,
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in
session this 9th day of January,2006 that it hereby approves a change to the City's health
insurance coverage effective February l ,2006 by replacing the current
CSD2500l000PS 103045C and the Premier Policy with additional employee co-pay and
deductible costs being self funded by the City of Waukee and administered by a Third
Party Administrator to the Choice Standard Policy PI 0200 1500POS80l 0]045,All
deductible and co-pay costs for services rendered after the effective date are to be borne
by the employees.The third party administrator under the previous program IS termmatcd
effective as ofthe 9th day ofJanuary,2006.
