RESOLUTION 10-230
APPROVAL OF A CONTRACT WITH LEISURE VISION RELATED TO A PARKS,
RECREATION AND OPEN SPACE COMPREHENSIVE PLAN NEEDS ASSESSMENT
SURVEY
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State ofIowa,is a duly organized Municipal
Organization;AND,
WHEREAS,the Waukee Parks and Recreation Board has determined that a Needs Assessment
Survey should be completed in connection with the City of Waukee Parks,Recreation and Open
Space Comprehensive Plan;AND,
WHEREAS,the Waukee Parks and Recreation Board reviewed the proposal submitted by
Leisure Vision for a Needs Assessment Survey Project in connection with the City of Waukee
Parks,Recreation and Open Space Comprehensive Plan;AND,
WHEREAS,the Parks and Recreation Board recommends that the contract attached as Exhibit
A submitted by Leisure Vision be accepted;AND,
WHEREAS,the City Attorney has reviewed the proposed contract and finds it satisfactory.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee,Iowa in
session this 9th day of August,201 0 that the contract submitted by Leisure Vision to complete a
Needs Assessment Survey is hereby accepted and approved.

PROJECT APPROACH-SCOPE OF SERVICES
Overview
Leisure Vision will administer a reliable and valid Needs Assessment Survey (Needs
Assessment)for the City of Waukee (City)for use by the City as part of the project team lead by
Genus.
The following Scope of Services identifies the tasks Leisure Vision will take.
1.Ron Vine,Leisure Vision's project manager,will begin working with representatives
from the City of Waukee and members of the Genus team to develop the survey and
ensure the questions address key issues.
2.Leisure Vision will complete 300 surveys.Results for the entire sampling of 300
households within the City of Waukee will have a 95%level of confidence with a
margin of error of +/-5.8 overall.Leisure Vision will guarantee completion of at
least 300 surveys for the needs assessment within the City of Waukee.Should we
receive more surveys those will be processed at no cost to the City.
3.Leisure Vision is capable of administering the survey entirely by phone or through a
combination of mail and phone.The costs for administering the survey by phone
only,or a combination of mail/phone are the same.
4.Questions on the survey will be developed in partnership with City staff ancl members
of the Genus team.It is anticipated that the survey will be up to five (5)pages in
length,including demographic questions or a phone length of 10 minutes.This
length will allow for between 15-22 questions to be asked,many with multiple
components.Leisure Vision has extensive experience working with Cities in the
development of survey questionnaires.
5.Leisure Vision has an ongoing quality assurance program in place.This program has
been developed and refined through our experience with hundreds of studies that
involved the design and administration of surveys.Our quality assurance program is
directly monitored by Dr.Elaine Tatham.The program is designed to give clients
"error free"results,and all employees at Leisure Vision are directly involved in the
program.
The survey will be administered by Leisure Vision staff at their corporate facilities
including all aspects of mailings,phone calling,development of the database,data
entry,etc.Total quality control for the project will be under the supervision of Ron
Vine,Project Manager and Dr.Elaine Tatham.All phone callers and data processing
staff are in the same office complex as Ron Vine and Dr.Tatham and have worked on

dozens of parks and recreation projects.All survey data is maintained on-site for a
minimum of 5 years and then off-site.All data will be made available for additional
cross-tabular analysis by the City for one (1)year from the completion of the needs
assessment.The City of Waukee will retain one digital copy of the back -up data.
6.Leisure Vision will conduct up to eight (8)cross-tabular comparisons of survey
results by key demographic factors,such as gender,age of respondent,length of
residency,income,users/non-users of services,etc.The demographic factors to be
cross-tabbed will be selected by the City of Waukee in consultation with Leisure
Vision.
7.A draft needs assessment survey report and final report will be developed for review
by the City staff and members of the Waukee team.Inclusive will be an executive
summary of findings,graphs and charts,cross-tabular analysis by regions,gender,
etc.
Timeline:
It will take five (5)weeks to administer the survey after a survey questionnaire has
been developed.
Professional Fees:
Professional fees will be $7,250
Miscellaneous Provisions and Modifications of the Agreement
This agreement is subject to modification upon written agreement by both parties.
