RESOLUTION 10-313
APPROVING CHANGE ORDER NO.1 TO CONSTRUCTION SERVICES,INC.,FOR
THE WAUKEE PUBLIC WORKS FACILITY IN THE AMOUNT OF $157,481.65
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State ofIowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on March 8,2010,the Waukee City Council approved the adoption of plans,
specifications,form of contract and estimate of cost for the Waukee Public Works Facility
Project (Resolution #10-51);AND,
WHEREAS,Design Alliance,Inc.,architect for the Project,recommends the City of Waukee
accept Change Order No.1 as noted on the Contracts Sunnnary dated December 14,2010,and
which is attached hereto as Exhibit 'B.'
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in session this
20th day of December 2010 that it hereby approves Change Order No.1 to Construction Services,
Inc.,for the Waukee Public Works Facility in the amount of$157,481.65.
