RESOLUTION 11-203
APPROVING AN ONLINE SUBSCRIPTION AGREEMENT BETWEEN THE
CITY OF WAUKEE AND BACKFLOW SOLUTIONS,INC.(BSI),RELATED TO THE
ADMINISTRATION OF THE BACKFLOW TESTING PROGRAM
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State ofIowa,is a duly organized Municipal
Organization;AND,
WHEREAS,the City is required to have a cross-connection control program which includes
mandated backflow tracking;AND,
WHEREAS,the Public Works Department recommends contracting with a third party to
administer the backflow testing program;AND,
WHEREAS,Backflow Solutions,Inc.CBSI),has provided an Online Subscription Agreement
related to the project,which includes meeting with and training tester companies,mailing
notifications,ensuring all test reports are complete and accurate,and providing consulting
services on backflow related issues;AND,
WHEREAS,the City Attorney has reviewed the agreement and finds it satisfactory;
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Online Subscription Agreement between the City of Waukee and Backflow Solutions,Inc.,for
the Backflow Testing Program is hereby approved.
Passed by the City Council of the City of Waukee,Iowa,and approved the 7th day of November,
2011.
