RESOLUTION 12-188
APPROVING SUPPLEMENTAL AGREEMENT NO.1,FINAL DESIGN SERVICES,
FOR THE UTE AVENUE BRIDGE OVER JOHNSON CREEK PROJECT
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Iowa is a duly organized municipality withiu Dallas County;
AND,
WHEREAS,on October 10,2011,the City of Waukee City Council approved a Consulting
Engineering Contract between the City of Waukee and Calhoun-Bums and Associates related to
the Jolmson Creek Bridge Replacement Project for project administration and coordination,
preliminary design,hydraulics analysis,final design,and bid and construction phase services
(Resolution #11-196);AND,
WHEREAS,Calhoun-Bums and Associates submits a Supplemental Agreement No.1 related to
Final Design Services for the proj ect;AND,
WHEREAS,the proposed supplemental agreement has been reviewed and approved by the City
of Waukee City Attorney and Iowa Department of Transportation;
NOW THEREFORE BE IT RESOLVED by the Waukee City Council that the City of
Waukee hereby approves Supplemental Agreement No.1,Final Design Services,for the Ute
A venue Bridge over Johnson Creek Proj ect.
Passed by the City Council ofthe City of Waukee,Iowa,and approved the 20th day of August,
2012.
