RESOLUTION 13-024
APPROVING SUPPLEMENTAL AGREEMENT NO.2,BID AND CONSTRUCTION PHASE
SERVICES,UTE AVENUE BRIDGE REPLACEMENT OVER JOHNSON CREEK,
PROJECT NO.SBRM-8177(614)-8F-25
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Iowa is a duly organized municipality within Dallas County;AND,
WHEREAS,on October 10,2011,the City of Waukee City Council approved a Consulting
Engineering Contract between the City of Waukee and Calhoun-Burns and Associates (the "Engineer")
related to the Johnson Creek Bridge Replacement Project (the "Project")for project administration and
coordination,preliminary design,hydraulics analysis,final design,and bid and construction phase
services (Resolution #11-196);AND,
WHEREAS,on August 20,2012,the City of Waukee City Council approved a Supplemental
Agreement No.I with the Engineer related to Final Design Services for the Project (Resolution #12-
188);AND,
WHEREAS,Calhoun-Burns and Associates submits a Supplemental Agreement No.2 related to the
bidding and construction phases of the Project;AND,
WHEREAS,the proposed supplemental agreement has been reviewed and approved by the City of
Waukee City Attorney and Iowa Department of Transportation;
NOW THEREFORE BE IT RESOLVED by the Waukee City Council that the City of Waukee
hereby approves Supplemental Agreement No.2,Bid and Construction Phase Services,Ute Avenue
Bridge Replacement over Johnson Creek,Project No.SBRM-8177(6l4)-8F-25.
Passed by the City Council of the City of Waukee,Iowa,and approved e 21 st day ofJanuary,2013.
