RESOLUTION 16-519
APPROVING PROFESSIONAL SERVICES AGREKMKNT AMENDMENT NO.2
WITH GENUS LANDSCAPE ARCHITECTS [FOX CREEK PARK IMPROVEMENT
PROJECT,PHASE 1]
IN THE NAME AND BY THE A UTHORITY OF THE CITY OF WA UKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on October 19,2015,the Waukee City Council approved an Agreement for
Engineering Services with Genus Landscape Architects related to the Fox Creek Park
Improvement Project,Phase 1 (Resolution ¹15-370);AND,
WHEREAS,on August 15,2016,the Waukee City Council approved Amendment No.I to said
Agreement (Resolution ¹15-352);AND,
WHEREAS,the City proposes Amendment No.2 to the Engineering Services Agreement,
attached hereto as Exhibit A,to complete an As-built Site Survey to assist in the determination of
the amount of dirt that was moved across the site during construction.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Professional Services Agreement Amendment No.2 with Genus Landscape Architects [Fox
Creek Park Improvement Project,Phase I]is hereby approved.
Passed by the City Council of the City of Waukee,Iowa,and approved the 19'"day of
December,2016.

Amendment to Contract ¹2.Fox Creek Park Phase I
Submitted:December 12,2016
I,This amendment (the "Amendment")is made by Genus
Landscape Architects.P.C.and City of Waukee.parties to the
Agreement for Protessional Consulting Services Fox Creek
Creek Park Phase I,dated October 19,2015.
2.The Agreement is amended to include the following
additional services to "3.Compensation".
K.As-built Site Survey $3,500
Total added to agreement $3,500
The total agreement amount shall be revised to:$70,850
3.Except as set forth in this Amendment,the Agreement is
unaffected and shall continue in full force and effect in
accordance with its terms.If there is conflict between this
amendment and the Agreement or any earlier amendment,
the terms of this amendment will prevail.
