APPROVING ENGINEERING SERVICES AMENDMENT NO.1 WITH FOTH
INFRASTRUCTURE k ENVIRONMENT,LLC [SE WKSTOWN PARKWAY,SE
PARKVIKW CROSSING DRIVE TO GRAND PRAIRIE PARKWAY]
IN THE NAMEAND BY THEA UTHORITYOF THE CITY OF 8'AUKEE,108'A
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on June 2,2014,the Waukee City Council approved an Agreement for Professional
Consulting Services with Foth Infrastructure and Environment,LLC,for engineering services related
to the SE Westown Parkway,SE Parkview Crossing Drive to Grand Prairie Parkway,Project
(Resolution ¹14-148);AND,
WHEREAS,Foth Infrasnucture and Environment,LLC,has provided an Amendment No.I to the
agreement to include additional services associated with project coordination,design services,final
plans and specifications for second plan set,bidding services for second project letting,and
construction administration for second project letting;AND;
WHEREAS,the engineering services agreement amendment has been reviewed and approved by the
Waukee City Attorney.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Engineering Services Amendment No.I with Foth Infrastructure k Environment,LLC [SE Westown
Parkway,SE Parkview Crossing Drive to Grand Prairie Parkway],attached hereto as Exhibit A,is
hereby approved.
Passed by the City Council of the City of Waukee,Iowa,and approved the 3'day of January,2016.

The City and Consultant agree to amend the original Agreement to include additional services associated with the
creation of a second independeni construction project for the extension of SE Westown Parkway as follows:
~Additional Project Coordination
~Additional Design Services to incorporate adjacent Development Agreements
Final Plans and Specifications for Second Plan Set
~Bidding Services for Second Project Letting
~Construction Administration for Second Construction Project
TASK A —Additional Project Coordination
The Consultant will complete the following project coordination as part of the project.
Task A 1 Periodic Meetin s with Cit Staff
Informational update presentations will be provided to the City Staff on an as needed basis during the Final
Design process and Construction.The presentations will provide an introduction/overview of the project and
project goals and objectives.It will also provide an update on design development concepts and schedule.For
estimating purposes,it is assumed that the Consultant will attend one (1)meeting every month for 12 months.
Task A.2 Exhibit Pre aration
Exhibits for the project meetings will consist of aerial mosaics for the project with the major project features
indicated on the aerial photograph.Include coloring,lettering,and other techniques to delineate the
proposed design concepts and right of-way needs,including typical sections of the roadway.The figure will
indicate right-of-way needs,property lines,property ownership,access control lines,grading limits,and new
project improvements.
Task A 3 Pro'ect Coordination with Ad'geant Landowner's En ineerin Consultant
The Engineer will conduct meetings to address specific conflicts and concerns.It is anticipated that there will
be two (2)meeting(s)with representatives of the adjacent Landowner.The first will be to advise of the nature
and extent of the improvements and any potential conflicts with existing or proposed utility systems,and the
second will be preliminary/final design review and coordination meetings.
Task A Subtotal —$17,000.00
TASK 8 —Additional Design Services to incorporate adjacent Development Agreements
After SE Westown was designed the following items were requested by adjacent property owners.
Task B 1 Additional To o ra hic Surya
The Consultant shall perform the following additional topographic surveys to determine the amount of
additional earthwork needed.This information will be used to develop the existing roadway DTM for the
Project site
~Silt basin located in the northeast corner of SE Parkview Crossing Dr.and SE Westown Pkwy.
~SE Westown Corridor to establish how much fill material has been brought in from nearby
developments.

Task B 2 Additional Plattin Services
Prepare acquisition plate and legal descriptions for property to be acquired for the Project and perform land
corner and lot surveys as needed for the preparation of the acquisition plate and legal descriptions for each
parcel.Said plate and legai descriptions shall comply with requirements of the lowe Code and shall be
prepared by or under the direct supervision of a licensed land surveyor under the laws of the State of lowe.The
following numbers of plate are assumed:

This task also consists of staking the easement locations for purposes of right-of-way negotiations.The
staking survey includes the marking of key easement location points in order to visualize the locations in the
field and shall also include marking of existing property lines locations.For the purpose of this agreement,
all permanent and temporary easements will be staked ~one 1 time.

TASK C —Final Plans and Specifications for second plan set
Based upon the approved revised preliminary design,field review,and property owner review,the Consultant shall
subsequently proceed with final design,contract drawings,specifications and opinion of probably construction costs for
the award of a single Contract for the construction of the proposed improvements.The work tasks to be performed
include the foliowmg;
Task C.1 Final Plans
The Consultant will perform final design services in the preparation of design plans and specifications depicting
the proposed grading,drainage,paving,signing,utility relocation,and other features of the project.The
preliminary plans will include but not be limited to the following tasks:
Task C 1 1 Final Title Sheets A Sheets
This task consists of finalizing the title sheet The title sheets will include the following Index of
sheets,Legend,Location Map,Project Number and Design Traffic Data
Task C 1 2 Final T ical Sections B Sheets
This task consists of final design and drafting of typical cross sections and standard details to be
utilized for the improvements
Task C 1 3 Final Quantities C Sheets
This task consists of the final bid items to be included in the Project,as well as final quantity
tabulations and the development of the general notes and estimate reference information.This item
also includes the final design and drafting of erosion control measures to be provided on the Project.
Task C 1 4 Final Plan and Profiles D and E Sheets
This task consists of the final design and drafting of roadway plan and profile sheets,including the
detail information required for plan approvals,permitting,and construction of the proposed
improvements.This task also consists of the final design and drafting for the installation of new
utikties and utility adjustments that can be determined from coordination wilh the utility companies at
the time of design.The scale af these sheets will be 1"=30'

This task consists of developing a suitable plan for construction scheduling and staging of the Protect

Task C.1 6 Final Markin s and Traffic Si nin J Sheets
This task consists of the development of pavement marking and traffic signing plans to be placed into
service following construction.The traffic control devices,procedures,and layouts shall be as per the
IVlanual on Uniform Traffic Control Devices (MUTCD).
Task C 1 7 Final Intersection Geometries L Sheets
This task consists of developing and refining the geometric layouts and provides additional horizontal
and vertical survey information needed to construct the intersections.The scale of these plan sheets
will be 1"=202
Task C.1.8 Final Desi n of Storm Drains e S stems M Sheets
This task consists of final design and drafting of storm sewers,storm sewer inlets,manholes,open
ditches,culverts,and other storm drainage related facilities for the Project.
Task C 1 9 Final Traffic Si nal and Fiber Desi n N Sheets
This task consists of final design of the interconnect layout and necessary details.This task will
include signal modifications at the Grand Prairie Parkway intersection to include necessary detection
on SE Westown Parkway.
Task C1 10 Final Roadwa Li htin Plan P Sheets
This task consists of final design and drafting of the photometric plan and locations of poles on the
drawings.The plans will include general layout features,including pole locations,handholes,
transformers,wiring/conduit locations and circuit design.
Task C 1 11 Final S ecial Construction Details U Sheets
This task consists of final design and drafting associated with special project details.Spemal
construction details include:
~Special Grading at the northeast corner of SE Westown Parkway and SE Parkview
Crossing Drive.
~Special Grading adjacent to the four weir structures.
Task C 1 12 Final Structural Plans V Sheets
This task consists of final design,calculations and drafting of the proposed weirs and cross-road
culverts including situation plans and necessary construction details to accommodate weir and culvert
construction.
Task C 1 13 Detailed Cross Sections W Sheets
This task consists of the design and drafting associated with the assembly of detailed cross sections
(50'increments)along SE Westown Parkway to illustrate typical conditions,drainage designs,and
non-typical conditions as needed for guidance during design,review,and quantity estimating
purposes.
Task C.2 Pro'ect Manual
This task consists of providing a Project Manual for the Project,including Notice of Hearing and Letting,
Instruction to Bidders,Bid Forms,Bond Forms,Agreement Forms,General Conditions of the Construction
Contract,Supplementary General Conditions,and Detailed Construction Specifications.

TASK D —ADDITIONAL PROJECT BIDDING
The Consultant will coordinate and manage the letting process for the project.For estimating purposes,~one 1
letting has been included as part of this task.The work tasks to be performed or coordinated by the Consultant shall
include the following:
Task O.1 —Printin of Plans and S ecifications
This task consists of printing and handling of the Plans and Specifications.For budget purposes it is estimated
that fifty (50)half-size (11"x17")plan sets and specifications will be duplicated and assembled by the
Consultant.This task also includes dissemination of the contract documents and maintaining a plan holders list
during the bidding phase.
Task D.2 —Notice of Pro'ect
The Consultant shall prepare of the formal Notice of Hearing and formal Notice of Letting publications.The City
shall handle publication of the Notice of Hearing and posting of Notice of Letting.The Consultant shall also
prepare and disseminate a Notice to Letting to the contractors concerning the upcoming Pro/ect.
Task D.S —Plan Clarification and Addenda
The Consultant shall be available Io answer questions from contractors prior to the letting and shall issue
addenda as appropriate to interpret,clarify or expend the bidding documents.
Task O.4 —Lettin Bid Tabs and Award Recommendation
The Consultant shall be a representative present when the bids and proposais are opened,shall make
tabulations of bid for the Owner,shall advise the Owner on the responsiveness of the bidders and assist the
Owner in making the award of contract,including preparation of necessary contract documents.

TASK E —ADDITIONAL CONSTRUCTION ADMINISTRATION
The original Agreement assumed one construction project estimated to be 16hrs /week of effort during the 2015
construction season.The magnitude,complexity and number of projects changed based on original
assumptions in 2014 Currently a second project is being constructed that will require construction
Administration Services during the 2017 construction season and final project close out in late 2017.
Additional Construction Administration efforts over and beyond the original effort includes additional
preconstruction meetings,review of construction shop drawings,field visits,construction meetings,processing
of pay vouchers,daily communication and modification of construction documents .The fallowing assumptions
are included:
~Average 12hrs/week beginning in the Spring of 2017 thru Fall 2017
Note —hours were reduced during winter shut down.Additional hours included to provide recorded drawings for
each project
Task E Subtotal -$45,000.00
TASK F —ADDITIONAL RESIDENT CONSTRUCTION OBSERVATION
The original Agreement assumed one construction project estimated to be 45hrs /week of effort during the 2015
construction season.The magnitude,complexity and number of projects changed based on original
assumptions in 2014.Currently a second proiect is being constructed that will require Resident Construction
Observation Services during the 2017 construction season.The following assumptions are included:
~Average 48hrs/week beginning in the Spring of 2017 thru Fall 2017

Consultant under this Contract Amendment of the original project.

jn consideration for these services,the City agrees to adjust the compensation for services performed by the Consultant.
These fees are based on the standard hourly rates of the Consultant's personnel actually engaged in the performance of
the services,plus direct out-of-pocket costs for expenses incurred by personnel who are actually engaged in the work
and other direct costs.The total Hourly Not-to-Exceed fees for the professional engineenng services as described in this
