RESOLUTION 17-386
APPROVING BUSINESS ASSOCIATE AGREEMENT (BAA)WITH
COMMUNICARE TECHNOLOGY,INC.,D/B/A PULSARA
IN THE NAME AND BY THE A UTHORITY OF THE CITY OF 5'A UKEE,IOV'A
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on August 14,2017,the Waukee City Council approved a Subscription Agreement
with CommuniCare Technology,Inc.,d/b/a Pulsara (Resolution //17-385);AND,
WHEREAS,implementation of the Pulsara communication system requires a confidentiality
agreement related to the transfer of protected health information between the Waukee Fire
Deparment and Pulsara,both of which are HIPAA-covered organizations.
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee,Iowa on
this 14"day of August,2017 that the Business Associate Agreement (BAA)with CommuniCare
Technology,Inc.,d/b/a Pulsara is hereby approved.
