APPROVING SUPPLEMENTAL AGREEMENT FOR ADDITIONAL SERVICES WITH
SNYDER &ASSOCIATES,INC.[WINDFIELD/HIGH SCHOOL TRAIL PHASE 1
PROJECT]
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOIYA
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on May 8,2017,the Waukee City Council approved an Agreement for
Professional Consulting Services with Snyder /k Associates,Inc.related to engineering services
for the Windfield-High School Trail Project (Resolution ¹17-232);AND,
WHEREAS,Snyder Ii'c Associates,Inc.,has since provided the City with a Supplemental
Agreement for Additional Services,attached hereto as Exhibit A,related to construction
administration servised;
NOW THEREFORE BE IT RESOLVED by the City Council of the City of Waukee that the
Supplemental Agreement for Additional Services with Snyder &Associates,Inc.[Windfield/
High School Trail Phase 1 Project]is hereby approved.
Passed by the City Council of the City of Waukee,Iowa,and approved the 18"'day of
September,2017.

DESCRIPTION OF ADDITIONAL SERVICES:
Upon award of the construciton contract,Snyder 8 Associates,Inc.wtll perform conslruclion
adminislraiion services for lhe project inckiding:
1.Proconslruction Conference;Snyder 8 Asso iates,Inc.will arrange and conduct a preconstruction
conference wilh Ihe Contractor,Subcontractors angl Ihe City to review the contract requirements,details
of construction,utility conflict and work schedule prior lo construction.
2.Contractor Paymenl Requesls;Requests for progress paymenls will be rewewed and approved based
on site observations,which authorizes payments and is a declaration tiiat ihe contractor's vvork has
progressed to the point indicated.
3.Shop Drawings:Shop drawings,if any and other submissions o[tlm Contractor will be reviewed for
general compliance with the construction contract.
4.Change Orders:Change Orders will be prepared for approval by the City.
5.Substantial Completion Final Site Obsorvationt 'Snyder 8 Associates,Inc.shall perform a site
observation to determine if ilia project ts subsfaniially complele according to ihe plans anti specifical:ons
and make recommendation on final paymcni.
Lump Sum in lhe amount of:5
8 Hourly in accordance with the original agreement fee schedule,or attached,with not to excood
budget of:$4,900
