APPROVING MIDAMERICAN ENERGY COMPANY AGREEMENT
[STREET LIGHT INSTALLATION)LAUREL ST —WMIS 2689658]
IN THE NAMEAND BY THEAUTHORITYOF THE CITY OF WAUEEE,IO8'A
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,MidAmerican Energy proposes an agreement,attached hereto as Exhibit A,to
install two (2)street lights along Laurel Street;AND,
WHEREAS,City staff recommends approval of the agreement.
NOW THEREFORE BK IT RESOLVED by the City of Waukee City Council in session this
16'"day of July,2017,that it hereby approves the MidAmerican Energy Company agreement
[Street Light Installation,Laurel St —WMIS 2689658].

Enclosed is ivfidAmerican Energy's proposal for the installation of'(2)nev'streetlights.
This proposal includes the installation ol'streetlights on:
Laurel St —WMIS 2689658
We Propose To:
install:LED equivalent lights on 28'galvanized
steel oles
These lights v ill be installed on steel poles and served by underground conductors,but
will be billed to the city at the wood pole,overhead rate because the developer has agreed
to pay the streetlight facilities charge.The applicant charge for this work is 5 10,006.60.
MidAmerican Energy Company's street light proposal is based on the approved street
light analysis provided by the City.MidAmerican Energy Company will install the lights
based on the staked locations provided in the field by the customer.
lf this proposal meets with your approval,please sign and return one (1)copy of this
letter and one (I)copy of the print.Please include payment of $10,006.60.
MidAmerican Fnergy Company will release the work upon receipt of the signed proposal
and payment.The other copy is for your file.

Upon receipt of your approval,we will prepare v ork instructions and schedule the
installation ol'these lights.This proposal is valid for 90 days and if MidAmerican Energy
Company construction has not commenced within l2 months it may be voided.
