RESOLUTION 14-140
APPROVING CONSULTING AGREEMENT &BUSINESS ASSOCIATE CONTRACT
WITH HOLMES MURPHY &ASSOCIATES,INC.
IN THE NAME AND BY THE AUTHORITY OF THE CITY OF WAUKEE,IOWA
WHEREAS,the City of Waukee,Dallas County,State of Iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,on June 2,2014,the Waukee City Council approved consulting agreement and
business associate contract with Holmes Murphy &Associates,Inc.,related to employee benefit
services;AND
WHEREAS,the State of Iowa Insurance Division has requested all sub broker fees be paid
direct to the sub broker;AND
WHEREAS,Holmes Murphy &Associates,Inc.,who has provided these services to the City of
Waukee since joining IGHCP in 2008,will now begin billing the fees direct to the City of
Waukee;
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in session this
2nd day of June,2014,that it hereby approves the consulting agreement and business associate
contract with Holmes Murphy &Associates,Inc.
