RESOLUTION 14-255
APPROVING AN AGREEMKNT WITH MODUS FOR THK DESIGN OF THK REPAIRS
FOR THK HVAC SYSTEM AT THK WAUKEE PUBLIC LIBRARY
IN THE NAME AND BY THE A UTHOIUTY OF THE CITY OF WA USE,
IONIA
WHEREAS,the City of Waukee,Dallas County,State of iowa,is a duly organized Municipal
Organization;AND,
WHEREAS,the Waukee Public Library has experienced extremely lugh utility costs and unreliable
HVAC service as a consequence of inefficient design;AND,
WHEREAS,City staff recommends contracting with Modus in order to design necessary repairs to
the Waukee Public Library HVAC system;AND,
WHEREAS,the Waukee Public Library Board of Trustees has reviewed the proposed agreement,
attached hereto as Exhibit A,and finds it satisfactory.
NOW THEREFORE BE IT RESOLVED by the City of Waukee City Council in session this 15"'
day of September,2014,that it hereby approves an agreement with Modus for the design of the repairs
for the HVAC system at the Waukee Public Library,subject to addressing issues identified by the City
Attorney.

RE:Waukee Library HVAC Updates
Dear Bill:
I am pleased to offer you this engineering service proposal for the design of an operational
Variable Air Volume (VAV)system with local zone thermal control.This proposal includes
contract documents,bidding assistance,submittal reviews and on-site meetings with construction
personnel as required.
Scope of design services includes the following:
~Redesign existing ductwork and diffusers as necessary to rezone system airflow
~Design new Variable Air Volume (VAV)terminal units with electric reheat where
needed and associated controls and thermostats
~Replace Air Handling Unit (AHU)Single Zone VAV controls with standard VAV
controls
~I-IVAC Controls
Evaluate new and existing power requirements
Design insulation For exterior ductwork (Alternate)
Design supplemental electric heat at perimeter windows (Alternate)
This proposal does not include the following services:
~Plumbing design
Ligh.ting design
~Fire alarm design
~Low voltage design
~Structural engineering
~Architectural modflciations
~Fees,permit costs or other costs associated with review or applications to governing
bodies
~As-built CAD drawing creation
Distribution and document reproduction expenses
~Life cycle cost analysis for utility rebates or energy code approval
~Commissioning services

For the above-referenced services,our fee will be lump sum,not to exceed $24,000.The
estimated construction budget is approximately $190,000 for a total cost of approximately
$214,000.
Additional services,as approved and requested,shall be provided at our hourly rates.
IvlODUS Errors and Omissions Policy shall be deemed a part of this Agreement.
The AIA 8101 (2007)Standard Form of Agreement Between Owner and Architect shall be
deemed a part of this letter.
